We are looking for a detail-oriented and patient-focused Population Health Medical Assistant to join our team! The Population Health Medical Assistant’s primary responsibility is to administer care management and coordination activities for the patients of the LMH Health primary care practices. This will include developing and monitoring care management and care coordination processes and support primary clinical teams with these efforts.

RESPONSIBILITIES:

  • Works with clinical teams as a care management resource overseen by an RN. This would include the following:
  • Involves the patients in activities to improve their health (patient engagement) and educates the patient about self-management tasks they can undertake to gain greater control of their health status
  • Maintains strict confidentiality; follows HIPAA regulations
  • Treats staff, physicians, NPs/PAs, visitors, patients and families with dignity and respect
  • Participates in professional development activities and departmental committee activities
  • Participates in pre-visit and post-visit planning to support coordination of care whenever possible
  • Performs other related work as required, actively manages assigned panel of patients in need of chronic condition support under the supervision of an RN:
  • Telephonic outreach to patients for care management/coordination services
  • Develops relationship with patients as an integral member of team
  • Provides follow-up contact with patient as indicated to ensure compliance with recommendations – medications, lab/x-ray, specialist visits, PCP visits, dieticians, CDE, etc.
  • Supports management of many aspects of the patient’s care: referrals to specialists, hospitalizations, ER visits, ancillary testing, and other enabling services; 
  • Responsible for being available to provide telephone advice per protocol
  • Anticipates the needs of this patient population, seeing that necessary documentation and pre-visit planning is completed or requested before patient visit;
  • Assists patients in setting goals for self –management, supports them in completion of self-management tasks and reports abnormal findings to their care team;
  • Collaborates with the patient, physician, and other care team members in assessing the patient’s progress toward individual health care goals;
  • Assesses barriers when patient has not met treatments goals, is not following treatment plan of care, or has not kept important appointments;
  • Collaborates with internal and external resources for additional services when appropriate Consistent documentation of patient self-management measures, via mutually agreed upon care plan that is efficiently available to all and reporting of progress towards goals
  • Assumes responsibility for patient safety by utilizing appropriate channels to communicate patient safety and patient care issues to appropriate bodies.
  • Maintains accurate and timely documentation within one business day of occurrence.

QUALIFICATIONS:

Required:

  • Completion of nationally recognized Medical Assistant program OR
  • Relevant experience and completion of a medical assistant program within 1 year of hire
  • BLS upon hire

Preferred:

Certified/Registered as a Medical Assistant

Six months experience in an outpatient clinical setting
