Job Summary

Palliative care is specialized medical care for people living with a serious illness with a focus on providing relief from the symptoms and stress of progressive illness. The advance practice provider works autonomously as well as in collaboration with physicians and other members of the health care team to diagnose and manage a patient’s health care problems. The advance practice provider will support physical, emotional, and spiritual well-being for patients and their loved ones. Consults are provided across the continuum of care i.e. hospital, clinic, nursing home and home settings.

Hours: M-F 8-430 (hours will vary), Position is 0.75 FTE

Essential Job Responsibilities

  1. Perform complete history and physical examination and documentation
  2. Utilize assessment data to plan and implement a plan of care in accordance with patient’s goals of care. Services include, but are not limited to, ordering, conducting and interpreting appropriate diagnostic and laboratory tests, prescription of pharmacological agents and treatments and non-pharmacological therapies
  3. Skilled in communication with patients, families, and other team members
  1. Able to work effectively within a diverse community, with a wide range of religious and cultural beliefs
  2. Capable of working collaboratively with clinicians and other members of interdisciplinary team
  3. Knowledgeable about current best practices in the field of hospice & palliative medicine
  4. Proficient in complex symptom management for serious illness
  5. Dedicated to improving the care of seriously ill patients and their families
  6. The care delivered will be based on theoretical principles and scientific knowledge and within the scope of the Kansas Nurse Practice Act

JOB QUALIFICATIONS
Required:

  • Graduate of an accredited training program for Physician Assistants or Nurse Practitioners
  • Current Basic Life Support for Healthcare Providers
  • Two years clinical experience

Preferred:

  • Prior experience in Hospice/Palliative Care
  • Advanced certification in Hospice/Palliative Care
