At LMH Health we are looking for a patient-focused, detail oriented, RN Maternity Navigator to join our team. The Maternity Navigator manages and serves as a liaison for patients in the Lawrence OB/GYN practices. The Maternity Navigator position is perfect for a mission and service driven RN with a passion for building relationships and advocating for their patients to ensure optimal outcomes and respectful maternity care.
Responsibilities
- Serves as a liaison between the patient, provider, and community referral agencies to ensure optimal health outcomes and achieve quality measures.
- Performs needs-based assessment of clients.
- Collects specific data, assesses the client’s condition, provides accurate consistent information, formulates and recommends a plan, negotiates appropriate options with the client, and validates the plan with the client for completeness and accuracy.
- Serves as a resource for patients based on the patient’s self-care knowledge deficits or non-compliance with prescribed plan of care. Must have OB and lactation knowledge base.
- Directs patients to the appropriate referral agency based on the needs assessment.
- Assists in scheduling appointments for client, as needed, with appropriate provider and services and encourages appropriate use of LMH resources and services.
- Acts in the role of client advocate to assist them through the health care system and community resources.
- Reports pertinent observations and reactions regarding clients to the appropriate person (i.e. primary care provider, social work, or Director).
- Maintains a patient centric, customer-driven professional attitude. Demonstrates awareness of own values and attitudes and how they may affect information given to a client. Provides non-judgmental advice based on protocols and resources.
- Collects and updates demographic information, as appropriate and ensures that complete and accurate documentation is recorded in the computerized database for each contact.
- Functions independently and demonstrates leadership in decision making, communication, and problem solving techniques to resolve clients’ questions/concerns.
- Collaborates with Social Worker, healthcare providers and Director as needed.
- Maintains excellent communication skills, chronic Obstetric and newborn knowledge base, proficient with computer applications and good problem-solving skills.
- Will serve as a representative for the Family Birth Center and LOGS by attending LMH community classes that are related to pregnancy.
- Will provide tours as needed for patient population.
- Will continue to obtain information from all patients prior to delivery so the appropriate information is available before hospitalization.
- Performs other duties as assigned.
Qualifications
Required:
- Current Registered Nurse license to practice in Kansas.
- Current Basic Life Support for Healthcare Providers (BLS).
- Minimum 3 years of RN experience in an acute setting.
Preferred:
- Bachelor of Science in Nursing (BSN).
- 5 years of obstetric RN experience in an acute setting.
