Position at Lifespark

The Life Care LPN works under the direction and supervision of a Life Care Manager, RN or Director, Life Care, RN. The LPN is primarily responsible for following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive client care outcomes. The Life Care LPN is responsible for promoting the Lifesprk experience to support proactive, whole-person care by delivering excellent client care and customer service to residents, families, visitors, and vendors.

Position Functions and Responsibilities

Client Care & Service Delivery

  • Provides direct client care as defined in the state Nurse Practice Act.
  • Implements plan of care initiated by the registered nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Monitors and provides client and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Assists with monitoring visits as directed by the RN Case Manager to evaluate ongoing client needs and quality of care
  • Completes nursing focused assessments during personal cares; document results; escalate to RN Case Manager as necessary for follow-up
  • Attends MD appointments as requested
  • Assists with skilled services as directed by care plan
  • Coaches, mentors, and supports unlicensed staff as needed.
  • Timely completion of required forms, reports, orders.
  • Assist clients with implementation Activities of Daily Living and Delegated Tasks according to clients’ needs, wishes, and preferences outlined on the Care Assignment
    • Review and implement Care Assignment
    • Performs Personal Cares (bathing, dressing, grooming, toileting, skin care, etc)
    • Lifting, Transferring (pivot transfer, 2-person, Hoyer lift)

    • Homemaking services
    • Vital Signs
    • Medication Reminders and Administration
    • Special Tasks
    • Fall Prevention
    • Observing and Reporting status updates and changes
    • Following all documentation requirement
    • Follows infection control and OSHA policies and procedures
  • Other duties as assigned by the RN Case Manager

Visits, Communication, and Customer Services

  • Lead by example in establishing rapport with clients, clinicians, partners, and the community.
  • Ensure positive/professional communication within the team
  • Customer service resolution; escalate concerns
  • Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.
  • Communicates with the physician regarding the client’s needs and reports any changes in the client’s condition; obtains/receives physician’s orders as required.
  • Communicates with community health related persons to coordinate the care plan.


  • Timely completion of all documentation as it relates to the care plan
  • Understands and complies with all the rules and regulations that govern the responsibilities of a Licensed Practical Nurse (LPN)
  • Completes orientation and annual training requirements as assigned
  • Participates in Lifesprk’s Event Management process including initiating reports, escalation of events and investigations
  • Promotes safety in the workplace through real time coaching and participation in safety awareness training
  • Instructs, supervises, and evaluates home health aide care provided every two (2) weeks
  • Manages client supplies assuring appropriate DME provider is utilized.
  • Other dudties as assigned


  • Prefer minimum of 1 year of nursing experience
  • Minimum of 1 year of caregiving experience (assisted living, home care, hospice)
  • Minimum of 1 year experience working with people in the second half of life.
  • Experience in customer service/direct member contact field preferred


  • Graduate of an accredited school of nursing
  • Current Minnesota Nursing License
  • Weekdays and every other weekend
