Responsible for ensuring the development, organization, and coordination of facility and community resources that are designed to meet the needs and interests of each resident to encompass the seven dimensions of wellness.

ESSENTIAL JOB DUTIES


  • Complete required documentation timely and accurately. Act as a Team Champion and mentor/coach/train caregivers and serve as Communication Liaison
  • Maintain community life systems as required by state/federal regulations

  • Communicate programs and develop positive relations with tenants, family, and volunteers
  • Coordinate and conduct team meetings and other meetings as assigned
  • Maintain department organization, including meeting deadlines
  • Foster Family and Community support through the Community Program i.e. newsletters, networking, activities that bring Family members and members of the community into the building
  • Contribute to the community efforts to maintain and/or improve quality of life and quality of care by participating in the following:
    • Care Conferences 
    • QAPI
    • Other meetings as assigned by supervisor
  • Provide orientation and training to staff as needed, including participating in new hire onboarding and in-service education
  • All other duties as assigned

QUALIFICATIONS

  • Bachelor’s degree in Therapeutic Recreation or activities related field is preferred
  • Professional experience in programming and management in a Memory Care Environment of at least 2 years is preferred
  • Ability to obtain a license to drive community bus
  • Must have a passion for working with older adults
  • Must be capable of maintaining regular attendance
  • Advanced computer skills
  • Must meet all local health regulations and pass a post-employment physical exam if required. This requirement also includes drug screening, criminal background investigation, and reference inquiry
  • Must be capable of performing the Essential Job Functions of this job, with or without reasonable accommodation
