The Geriatric Care Support RN is a valuable team client who works in collaboration with the NP/MD/LM team to provide client centered clinical care and support to clients in home based primary care and other Lifesprk Health programs. The Geriatric Care Support RN is an experienced nurse who is comfortable with medically complex geriatric clients with an emphasis on providing care in the home with exceptional attention to the client experience.  The Geriatric Care Support RN works in collaboration with other team clients involved to ensure seamless care delivery and communication.

Position Functions and Responsibilities

Client Care & Service Delivery

  • Reviews clinical assessment and care plan of client to carry out home based primary care needs at the direction of the NP/MD/LM. In some settings, may provide a complete physical assessment and history of current and previous illness(es) and establish a care plan.
  • Regularly re-evaluates client nursing needs via telehealth and in person.
  • Implements and oversees the plan of care and collaborates with MD/NP/LM to implement best course of intervention as client status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, or carry out care plan tasks developed by the MD/NP/LM which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions, or coordinates for additional services to meet client’s needs. Includes the client and the family in the planning process.
  • Prepares clinical notes with entry of client VS, weights, PMH, Immunizations, allergies etc.
  • In some instances, administers medications and treatments, IV fluids, performs venipuncture, obtains specimens and completes other skilled nursing tasks as prescribed by the NP/MD.
  • Counsels the client and family in meeting nursing and related needs. Instructs family in use of telehealth technology and process.
  • Provides health care instructions to the client as appropriate per assessment and plan of care.
  • Communicates consistently and clearly with NP/MD/LM via EMR. Monitors clinical inbox to assist NP/MD with obtaining lab/imaging results, communication from other specialists, place orders for prescribed medications.
  • Coordinates with vested partners and maintains a high standard of care coordination and follow through.

Visits, Communication, and Customer Services

  • Lead by example in establishing rapport with clients, clinicians, partners, and the community.
  • Ensure positive/professional communication within the team
  • Customer service resolution; escalate concerns
  • Prepares clinical notes and updates the primary physician when necessary and per regulatory guidelines.
  • Communicates with the NP/MD/LM regarding the client’s needs and reports any changes in the client’s condition; obtains/receives physician’s orders as required.
  • Communicates with community health related persons to coordinate the care plan.


  • Timely completion of all documentation as it relates to the care plan by 10am the next day
  • Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse
  • Completes orientation and annual training requirements as assigned
  • Participates in Lifesprk’s Event Management process including initiating reports, escalation of events and investigations
  • Promotes safety in the workplace through real time coaching and participation in safety awareness training
  • Manages client supplies assuring appropriate DME provider is utilized.

Time & Territory Management

  • Schedules and keeps client appointments
  • Organizes travel efficiently and documents mileage accurately within EMR

Other duties as assigned




  • One to two years of recent acute care experience in an institutional setting. One year of home health care or community-based healthcare experience preferred.
  • Experience working with seniors required.
  • Experience in customer service/direct client contact field preferred
  • Experience supervising caregivers


  • Graduate of an accredited school of nursing
  • Current Minnesota Nursing License
  • Current CPR certification


  1. Participates in on-call program rotation
  2. Participates in weekend and holiday rotation
  3. Strong phone communication skills
  4. Proficient with technology and technological applications: Microsoft Office, Smart Phone, Calendaring, Wi-Fi, EMR, video-based conferencing, Internet Browsing, Printer
