Responsible for overseeing and facilitating the successful scheduling, completion and submission of all necessary documentation (MDS) to ensure compliance and positive resident outcomes. The MDS Coordinator will monitor coding and documentation to ensure accuracy and conducts and coordinates ongoing resident assessments necessary for the accurate completion of the Minimum Data Set (MDS), Care Area Assessments (CAA), and the resident’s “Plan of Care”.

Essential Job Functions

  • Educate nursing staff on proper documentation to ensure quality of care, compliance, appropriate assessments, and interventions including POC training for nursing assistants and PCC clinical nursing assessment.
  • Ensure consistent, accurate and timely completion of the RAI process which includes MDS, Care Area Assessments (CAAs) and plan of care.
  • Work with the Interdisciplinary Care Team in developing a comprehensive resident assessment and care plan for each resident.
  • Verify required documentation is accurate and State and Federal reimbursement guidelines are met.
  • Determine Medicare/Managed Care eligibility on all admissions and readmissions.
  • Determine charting needs for Medicare/Managed Care Skilled covered residents, ensuring this information is available to nursing staff.
  • Coordinate scheduled Medicare/Managed Care meetings with other departments regarding continued coverage of Medicare A and Managed Care residents.
  • Review the status of continued coverage for Medicare A/Managed Care residents, including determination/coordination of in-lieu of days, MSHO days, etc.
  • Coordinate completion of Medicare denial letters with accurate information and inform the responsible party/ coordinate appeal process as necessary.
  • Obtain and coordinate physician certification on day of admission or as soon as possible thereafter and on specified intervals thereafter.
  • Review residents during their 30 day window to determine need for return to Medicare A/Managed Care coverage.
  • Weekly, or as determined by the Managed Care contracts, contact the Managed Care organization to determine continued coverage.
  • Transmission of MDS records and review of validation letters.
  • Coordination of case mix letters to the business office or other appropriate discipline to ensure timely delivery of case mix letters.
  • Review Medicare paperwork for accuracy.
  • Manage MDS schedule and open assessments prior to ARD.
  • Coordinate and assist with accurate and timely completion of resident specific clinical assessments.
  • Assist in identifying and assessing residents for significant change in status which may include when coming off skilled service or initiation of part B therapy.
  • Observe residents for changes in ADL scoring.
  • Keep unit manager informed of changes occurring with the resident’s plan of care.
  • Complete walking rounds on the unit on a daily basis.
  • Keep Director of Nursing Services informed of Medicare issues as necessary.
  • Maintain integrity and accuracy of weekly PPS log and Individual PPS logs.
  • Perform duties while observing resident’s rights, providing dignity, privacy, and confidentiality.
  • Must be adaptable, flexible to changes, and able to prioritize and organize work efficiently to accomplish workload within time allotted.
  • Maintain positive relationships with key stakeholders such as tenants, their families, staff, and the community.
  • Maintain a positive attitude in relation to customer service program.
  • Complies with established safety rules and regulations.
  • Promptly reports accidents, incidents, and unsafe and hazardous conditions/equipment to supervisor.
  • Understands, complies with, and promotes all rules regarding resident’s rights.
  • Abides by the facility’s policies, procedures and practices.
  • Participates in and supports Quality efforts throughout the facility.
  • Attends meetings and mandatory in-services.
  • Knowledge and adherence to safety/disaster preparedness plan.
  • Attends training and maintain current nursing license and CPR certification.
  • All other duties as needed, whether assigned by the Consultants, Director of Nursing Services, or the Executive Director.


  • Valid state Registered Nursing licensure.
  • Minimum of one year experience in long-term care with a strong background in the MDS and regulatory compliance preferred.
  • Possesses a thorough understanding of the MDS, care planning, and medical record documentation standards.
  • Possesses an understanding of Medicare, Medicaid, and Insurance billing.
  • Working knowledge of computer hardware and software.
  • Knowledge of State and Federal regulations as they apply to long-term care.
  • Ability to be a functional member of an interdisciplinary problem solving team.
  • Ability to foster good interdepartmental relationships.
  • Good leadership skills and attention to detail.
  • Exceptional telephone and communication skills and the desire to work closely with senior adults is essential.
  • Must be able to relate professionally, positively, and cooperatively with residents, resident’s families and employees.
