Clinical Liaison LPN is primarily responsible for assisting with the implementation and execution of the Lifesprk Health consultative care services provided to our TCU and campus clients.  In this position you will be a part of an energetic, positive team that assists our Nurse Practitioners and Physicians in delivering exceptional primary care in our TCU, Assisted and Independent Living partnered sites. This position requires excellent communication and customer service skills, team player approach and ability to perform in a fast-paced environment. The ideal candidate would have experience with EMR’s such as Athena and PCC, computer software skills, highly organized and positive. Clinical experience in a nursing home/TCU/Assisted Living, Home Care or Clinic setting is required. This position will offer some direct patient care along with phone triage, communication to providers out on site, communication with facility staff, chart documentation, assistance with prior authorizations.

 

Position Functions and Responsibilities

Office Support and Customer Service

  • Perform excellent customer service to clients, customers, partners and colleagues daily
  • Answer and triage all incoming calls
  • Documentation of call details in EMR
  • Direct communication with MD/NP for urgent client needs.
  • Assist with scheduling/chart loading/medication reconciliation, vital sign entry.
  • Assist Independent living clients with urgent needs within the scope of practice of LPN/MA.
  • Participate in weekly meetings with team and monthly provider meetings.
  • Collaborate with NP/MD team and assist with exams/procedures as needed.
  • Provide coverage when needed for other clinic staff

Qualifications

  1. Licensed Practical Nurse with a minimum of 1 year of experience in community-based services (assisted living, TCU, LTC). Would consider clinic experience.
  2. Prefer experience in an administrative/office support role
  3. College degree in healthcare or business field preferred.
  4. Experience in customer services/direct client contact field preferred
  5. Experience using technology including Microsoft Office, Smart Phone, Calendaring, Clinical Software platforms (EMR), Wi-Fi, Printers, and Internet Browsing
  6. Ability to prioritize tasks based on need of clients and providers and other priorities.
