Lifesprk, a leader in holistic senior care, is looking for an advisor in senior care who can help overcome barriers within the existing healthcare system to identify and meet the unique needs of seniors in the Twin Cities metro. 

Using a value-based model in partnership with North Memorial Health and UCare, Lifesprk utilizes the 7 essential elements of wellbeing to improve the lives of seniors and spark lives.  This innovative program assists seniors to prevent re-hospitalization, address care gaps, and meet non-medical needs which are necessary to help seniors thrive.

The Life Manager is responsible to engage eligible members in the Lifesprk Experience™ through proactive outreach and the implementation of Lifesprk’s proven process.  He/she works one-on-one with members to discover what is most important to them and develop a customized Life Plan that addresses all 7 elements of well-being.   She/he manages the Life Plan, providing proactive guidance and connecting to essential 7 element services to deliver results.  Acts within the full capacity of her/his licensure. 

We are looking for an innovative and creative individual who wants to contribute a whole person experience for seniors outside of traditional healthcare models.  Candidates must be able to build relationships with members, families, and invested partners to engage them in the Lifesprk Experience (LSE).

Position Functions and Responsibilities

Client Experience

  • Proactive outreach to eligible members to engage them in appropriate services
  • Complete initial discovery:
    • Identifies need for Primary Care
    • Identifies need for Lifesprk and Lifesprk partner resources/services
    • Makes referrals to appropriate Lifesprk and Lifesprk partner services/resources
  • 1 to 1 meetings with members and families in the home
  • Coordinates transitions for members as appropriate
  • Develops and implements member Life Plan including:
    • Completes discovery to identify each member’s unique situation across the 7 elements of well-being
      • Health & Wellness
      • Thinking & Memory
      • Home & Safety
      • Finance
      • Identity
      • Purpose & Passion
      • Social Supports
    • Development and revisions of a tailored plan that addresses member’s needs, challenges and desired outcomes
    • Identification and coordination of resources to fulfill all areas of the member’s Life Plan
    • Serve as a member advocate and liaison working with outside resources
    • Provides ongoing education to member/family

Leads the Interdisciplinary Team

  • Collaboration with Primary Care, Lifesprk team members, partners and resources
  • Leads team meetings to celebrate member progress and achievements and solicit feedback from members, family members and/or others involved in member’s care


  • Timely completion of all documentation
  • Responsible for member life record content including discoveries, ongoing discoveries, life plans, outcomes and notes
  • Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse
  • Completes orientation and annual training requirements as assigned
  • Participates in Lifesprk’s Event Management process including initiating reports, escalation of events and investigations
  • Promotes safety in the workplace through real time coaching and participation in safety awareness training

Time & Territory Management

  • Schedules and keeps member appointments
  • Organizes travel efficiently and submits accurate auto use logs by established deadlines
  • Tracks all billable activities


  • Participates in on-call program rotation
  • Participates in weekend coverage as needed



  • Registered Nurse, BSN preferred
  • Prefer advanced certification as a Care Manager, Holistic Nursing, Geriatric CM, and/or Gerontology


  • 2 years’ RN experience in community-based services (assisted living, home care, hospice, public health)
  • Experience working with seniors preferred
  • Experience in customer service/direct member contact field preferred
  • Experience in multiple settings and situations (trying to get at life experience – more than just home care)
