Full-Time Home Health Aide (HHA) - Guaranteed between 32-40 Hours/week! 

Scheduling options of: 8AM-6PM or 12PM-10PM (4 working days/week - with every other weekend rotation

Position Functions and Responsibilities                                                 

Lifesprk Experience

  • Assist with implementation of Lifesprk Experience including items identified on the Care Plan.
    • Review Care Plan and understand expectations.
    • Provide companionship and socialization.
    • Assist with activity and hobby fulfillment.
    • Communicate with families regarding client activities and engagement.
    • Provide transportation (as needed).
    • Other Duties as Assigned.
  • Assist clients with activities of daily living and delegated tasks according to clients’ care plan, needs, and wishes.
    • Review Care Plan and follows what is expected.
    • Personal Cares (bathing, dressing, grooming, toileting, skin care, etc.).
    • Lifting, Transferring (pivot transfer, 2-person, Hoyer lift).
    • Range of Motion & Positioning.
    • Exercise & Ambulation.
    • Meal Prep and assist with feeding (as needed).
    • Homemaking (light housekeeping, laundry, personal shopping).
    • Vital Signs.
    • Medication Reminders and Administration.
    • Special Tasks as directed by RN Case Manager.
    • Observes and Reports Client Status updates and changes.
    • Follows all documentation requirements.
    • Other Duties as Assigned.
  • Make the clients feel comfortable and well-cared for.
  • Manage your time; clock in and out promptly and accurately, complying with attendance policy.
  • Maintain accurate and timely documentation of tasks and pertinent client-related information.
  • Communicate any concerns, problems, or issues to Care Supervisor or RN Case Manager promptly.
  • Understand and follow all company policies, including infection prevention and control, and emergency preparedness and response.
  • Maintain privacy and confidentiality of client’s information.
  • Comply with compliance plan.

Employee Experience

  • Assists with Caregiver Orientation as needed.
  • Real-time coaching/mentoring for new caregivers.
  • Familiar with Call-in Protocol & Expectations; Assist as needed in emergency scenarios.
  • Attend regularly scheduled Staff meetings and contribute to positive and productive discussion regarding improving the client and/or employee experience.


  1. Prefer a minimum of 1 year of caregiving experience (Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice) or be an active nursing student.
  2. Certified Nursing Assistant, currently active on the MN CNA registry, preferred. If you aren’t currently a registered CNA, but meet all other qualifications for the job, we will reimburse you to become a registered CNA after 6 months of employment, granted you are still in good standing.
  3. Prefer High school diploma or equivalency.
  4. Minimum 18 years of age.
  5. Must have valid driver's license and be willing to drive within the defined territory.
  6. Ability to drive a car and have reliable transportation.
  7. Ability to read, write, and speak the English language.
  8. Ability to pass written and skill competency examinations.
  9. Ability to use a smart phone or tablet.
  10. Ability to cope emotionally and physically with wide range of client behaviors.
  11. Must be proficient in use of technology and be able to exhibit ability to download a scheduling/time keeping app and use a scheduling/time keeping app.
  12. Must be able to work with clients who have pets.

Guaranteed Hours Terms

  • Home Health Aide II agree to be available for 40 hours per week within their agreed upon schedule and will be guaranteed a minimum of 32 hours.
  • Home Health Aide II must be willing to work with any client, regardless of personal preference.
  • Home Health Aide II must be able and willing to work with clients of all skill levels.
  • Home Health Aide II must be willing to travel anywhere within their territory.
  • Home Health Aide II will be working under a guaranteed hours agreement. Thus, the Home Health Aide II will forfeit their guarantee for the week if they refuse a shift scheduled within their agreed upon availability.
  • Days off during regularly scheduled workdays will require submission of PTO days (i.e. days are not flexible).
  • Time traveled between job sites in the same day will be paid at the regular rate of pay
