Are you passionate about making a positive impact on the lives of individuals struggling with mental health disorders? Join our team at KMHS Mental Health Services as a Care Coordinator for our FACT (Flexible Assertive Community Treatment) program. As a Care Coordinator, you will play a vital role in providing essential support and guidance to clients on their journey to recovery.
Primary Responsibilities:
- Collaborate with the FACT team to provide comprehensive case management to assigned clients.
- Participate in individualized treatment planning for clients, focusing on their unique needs and goals.
- Motivate and engage clients in making positive behavioral changes to support their long-term recovery.
- Educate clients and their families about mental health diagnoses and the recovery process for psychotic disorders.
- Document client progress accurately and timely, showcasing the relevance of case management activities to treatment plan goals.
- Assist clients in developing essential skills for independent living, including daily tasks and accessing community resources.
- Contribute to crisis intervention efforts and coordinate with other team members, law enforcement, and crisis outreach teams when necessary.
Minimum Qualifications:
EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field.
EXPERIENCE: Entry level (no prior related work experience)
This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must meet agency driving requirements, including meeting the minimum age requirement of 21 years old, successfully completing a motor vehicle history check, possessing and maintaining a current, valid driver's license in the state of Washington, and having reliable, insured transportation.
LICENSURE: Agency Affiliated Counselor Registration. (Can be attained after hire)
Preferred Qualifications:
EXPERIENCE: Experienced (minimum 2 years of job-related experience)
Experience providing comprehensive case management to clients in a community mental health or social services setting.
