Hiring Range: $24.94 - $30.55 per hour

Kitsap Mental Health Services (KMHS) is seeking a dedicated Care Coordinator with a Bachelor's Degree in Psychology, social services, or a behavioral health field to join our Adult Outpatient Team. As a Care Coordinator, you will be an essential member of our multidisciplinary team, providing comprehensive care coordination, treatment, and support services to our clients.

Primary Responsibilities: 

  • Provide direct care coordination services to an assigned group of clients, including individual, group, and family counseling, crisis intervention, and case management.
  • Collaborate with the treatment team to develop, implement, evaluate, and revise treatment goals and plans for clients.
  • Educate and support clients' families, advocating for clients' rights and preferences. Coordinate with community agencies and systems to ensure holistic treatment.
  • Assist clients in finding primary care physicians, dentists, and coordinating psychiatric care with medical care.
  • Offer crisis intervention counseling and collaborate with Designated Crisis Responders when necessary.
  • Maintain thorough and accurate documentation of client progress according to established policies.
  • Participate actively in organizational meetings and treatment planning review meetings.
  • Provide information and support to clients regarding substance use issues, mental and physical health, and daily functioning.
  • Help clients access resources and education for increased autonomy and meeting basic needs.
  • Monitor and facilitate Legal Restraint Alternatives (LRA) compliance for assigned clients, ensuring continuity of care.
  • Collaborate with the Crisis Response Team (CRT) and court-involved personnel for LRA extensions and revocations as needed.

  

  

  

Minimum Qualifications:  

EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field. 

EXPERIENCE: Entry level (no prior related work experience) 

This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must successfully complete a motor vehicle history check, possess and maintain a current, valid driver's license in the state of Washington, and have reliable, insured transportation. 

LICENSURE: Agency Affiliated Counselor Registration.  

  

Preferred Qualifications: 

EXPERIENCE: Experienced (minimum 2 years of job-related experience) 

Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment. 

The capacity to work well with children and families.
