Youth Inpatient Unit Discharge Planner - 32 Hours Per Week
Hiring Range: $20.07 - $24.58 Per Hour

The Discharge Planner supports the multidisciplinary team by providing and tracking discharge-planning services that are complex, multi-system, and specific for the people we serve on the inpatient units. Having made use of various resources, the discharge planner assists the discharging person and their family if applicable with navigation of external resources. 


Primary Responsibilities:

  • Coordinate and facilitate timely discharge plan implementation for clients served on the inpatient units.
  • Develop and coordinate discharge plans that are: complex, multi systemic, mixed funding, and specific to clients.
  • Assist clients in attaining and maintaining safe, affordable housing and/or shelter by making the appropriate referrals.
  • Assist and support clients in finding a primary care physician, dentist, and other medical specialists as required.
  • Provide practical help and supports to help clients access other necessary services such as legal services, housing subsidies, and funding alternatives such as SSI, SSDI, and Veteran’s Administration benefits.
  • Track client’s progress upon discharge for no less than thirty (30) days after discharge from the facility.  Arrange follow-up care as appropriate.
  • Collaborate with the case management services department of the client’s insurance to provide appropriate referrals and follow-up appointments.
  • Maintain effective communication with physicians, nursing staff, individuals served, families and other related to discharge planning.
  • Coordinate with social services personnel to provide services as indicated by the treatment team. 
  • Document findings, discharge plans, and action taken on medical records according to -KMHS guidelines; prepare reports and maintain records as required.
  • Maintains the clinical information of individuals served by the unit in a confidential manner.
  • Supports the Mental Health Professionals in completing case management tasks associated with discharge such as arranging for shelter beds for our adult clients, as well as, follow up appointments for adults and youth.
  • Participate in treatment team meetings 3 times per week at the unit.
  • Track the total number of all discharges from the unit and differentiate between those deemed complex and those deemed standard.

 

Minimum Qualifications:

EDUCATION: High School Diploma or GED

EXPERIENCE: Entry level (minimum 1 year of job-related experience)

Valid Washington Food Handler card.(Can be obtained post-hire)

LICENSURE: Agency Affiliated Counselor Registration (or ability to obtain one post-hire)

 

Preferred Qualifications:

EDUCATION: A Bachelor’s degree in Psychology, Social Work or a Behavioral Health Field.

EXPERIENCE: Experienced (minimum 2 years of job-related experience)

Relevant experience working with clients with severe and persistent mental illness.

Capacity to work well with children and families.
