Hiring Range: $20.07 – $24.58 Per Hour

Kitsap Mental Health Services (KMHS) is hiring a Discharge Planner to join our Inpatient Services team. In this role, you will support the multidisciplinary team by coordinating and tracking complex discharge planning for individuals served on the inpatient units. The Discharge Planner helps clients and families navigate housing, medical care, social services, and benefits while ensuring a smooth transition from inpatient care back to the community. This position plays a vital role in supporting recovery and stability after discharge, and provides continuity of care by tracking client progress for at least 30 days post-discharge.
Primary Responsibilities:
- Coordinate and facilitate timely discharge plans for clients on the inpatient units, ensuring they are comprehensive, multi-system, and individualized.
- Assist clients in securing safe, affordable housing or shelter through appropriate referrals.
- Support clients in accessing primary care, dental, and specialty medical services.
- Provide practical assistance connecting clients to legal resources, housing subsidies, SSI, SSDI, Veterans benefits, and other funding sources.
- Track client progress for at least 30 days post-discharge, arranging follow-up care as needed.
- Collaborate with insurance case management to ensure appropriate referrals and follow-up appointments.
- Communicate effectively with physicians, nursing staff, clients, families, and treatment teams regarding discharge planning.
- Coordinate with social services and case management staff to meet client needs during transition.
- Document discharge plans, findings, and actions accurately in medical records, maintaining confidentiality at all times.
- Support Mental Health Professionals in discharge-related case management tasks, such as arranging shelter beds or follow-up appointments.
- Participate in treatment team meetings three times weekly to review client needs and progress.
- Track all discharges and identify those deemed complex versus standard.
- Maintain required reports, records, and training as per agency standards.
Minimum Qualifications:
EDUCATION: High School Diploma or GED
EXPERIENCE: Entry level (minimum 1 year of job-related experience)
Valid Washington Food Handler card.
LICENSURE: Agency Affiliated Counselor Registration
Preferred Qualifications:
EDUCATION: A Bachelor’s degree in Psychology, Social Work or a Behavioral
Health Field.
EXPERIENCE: Experienced (minimum 2 years of job-related experience)
Relevant experience working with clients with severe and
persistent mental illness.
Performance Requirements:
KNOWLEDGE:
1. Maintain a working knowledge and skill in verbal de-escalation skills as
outlined in MPAC/CPI trainings and other agency-mandated trainings
