POSITION:PSM Coordinator

DEPARTMENT:Maintenance

LOCATION:Ottumwa

Objective: To organize and maintain the Process Safety Management (PSM) program for Pilgrims ammonia refrigeration systems. Has ownership of all aspects of the PSM/RMP process and will work closely with all site personnel and leadership, to ensure site compliance with OSHA and EPA requirements as well as adherence to all company and site policies and procedures.

Specific Job duties/tasks

  •                    Maintain APSM data ensuring that all information is up to date, and that all folders and information contained therein have current information pertaining to all PSM program elements and subsequent action items and records.
  •                    Manage all file system records for applicable program elements, i.e., PM records, B-109 records, MOC records, Incident Investigation records, Employee Participation records, Training records, and all other such records generated from any PSM/RMP program elements.
  •                    Maintain Bulletin Boards with current PSM/refrigeration information, applicable incident reports, and other relevant PSM/refrigeration department information.
  •                    Schedule and participate in monthly refrigeration department PSM meetings – review all open action items, status of B-109 inspections, SOP refresher training, and other pertinent items, like MOC project updates and incident reports.
  •                    Participate in corporate facilitated PSM Information conference call – prepare to deliver one such meeting with support from corporate PSM manager.
  •                    Prepare the quarterly status report by updating all open recommendation tracking sheets with the refrigeration management. Schedule and participate in Quarterly PSM Review meeting.
  •                    Participate in 3-year Compliance Audits and 5-year Process Hazard Analysis, and maintain open recommendation tracking forms.
  •                    Work with 3rd party and Corporate PSM Manager to coordinate and complete the 5-year Mechanical Integrity Inspection. Maintain open recommendation tracking forms.
  •                    Participate in all government inspections including: OSHA’s NEP, PQV, and EMP’s RMP inspections. Assist with the gathering of all requested records, and program guidelines for associated PSM/RMP elements.
  •                    Have been trained in the following within 1st year of hire:
  •                    PSM/RMP course (outside company)
  •                    Hazmat certified (8 hour minimum)
  •                    APSM
  •                    Incident Command system certified (optional)
  •                    SAP
  •                    AutoCAD (optional)

PSM/RMP Detailed tasks

Employee Participation 

  •                    Ensure hourly employees are involved with the development and review of Process Safety Information
  •                    Ensure hourly employees are involved during the PHA and CA processes
  •                    Maintain PSM team meeting minutes and post (at least monthly)
  •                    Work with Safety Management to develop/provide information for annual PSM training of all site personnel
  •                    Maintain up to date PSM related activities on the maintenance bulletin boards
  •                    Utilize PAC-14 or develop other training to review on a monthly basis with all refrigeration operators/technicians.

Process Safety Information

  •                    Ensure refrigeration staff review P&ID’s annually, and review all valves for accurate valve tags and maintain documentation in APSM
  •                    Ensure refrigeration staff cycle ammonia valves annually
  •                    Ensure B-109 inspections are completed annually for each piece of equipment and maintain documentation in APSM
  •                    (Use the Annual Revalidation Sheet for the above) (assemble PM’s and file once closed)
  •                    Update the inventory calculations, equipment list, and valve list for site with assistance from refrigeration supervisor and maintain documentation in APSM


 

PHA Process Hazard Analysis –

  •                    Work with the Corporate PSM Manager to verify that the PHA revalidation is completed on time and includes all required personnel. Coordinate the logistics of the PHA. Participate on the PHA revalidation team.
  •                    Ensure the PHA report has been reviewed and signed off by each refrigeration operator/technician
  •                    Maintain the status of the open recommendations and have operators review after each monthly/quarterly meeting

SOP Standard Operating Procedures –

  •                    Maintain electronic versions of SOP’s in APSM
  •                    Ensure operators check and verify the SOP associated with each valve group/equipment are accurate and maintain documentation in APSM
  •                    Develop a training schedule to re-train each operator on all of the SOP’s at a minimum of every three years
  •                    Ensure that management certifies that the SOPs are current and accurate annually

Training–

  •                    Maintain all training records for each operator in APSM
  •                    Develop a schedule to ensure continual on-the-job training for each operator (work with refrigeration supervisor)
  •                    Ensure all newly hired operators follow the training guidelines established in our corporate training guideline
  •                    Maintain records of the training for each new operator on all of the SOP’s/MIPs (Initial Training) on APSM
  •                    Ensures the staff re-trains each operator at least every three years on all of the SOP’s/MIPs (refresher) and maintain documentation in APSM
  •                    Maintain records for the annual consultation with each operator to review their training needs and requirements using the corporate “Annual Consultation” template and maintain documentation in APSM
  •                    Ensure safety/medical has the following: annual PFT, annual Fit test, annual medical evaluation, respirator use training, LOTO, Fall protection, Hot work/welding, Confined space, forklift (where applicable), Hazard communication, 24-hour Hazmat technician level for new hires, and 8-hour Hazmat refresher annual (maintains the Training Matrix in APSM)

Contractors 

  •                    Obtain and maintain welding certificates for all outside contractors performing work on the Ammonia system in each MOC project file
  •                    Maintain welding examination reports for all work performed on the Ammonia system (where applicable)
  •                    Maintain the qualification documents for any contractor working on or around the Ammonia system (if not already provided in the ISN site)
  •                    Obtain and maintain any state licenses required for all outside contractors performing work on the Ammonia System in each MOC project file.

Pre startup Safety Review 

  •                    Schedule and lead the Pre-Startup Safety Review walk-through team before startup of any new/altered equipment. Verify all walk through team members sign off on the PSSR paperwork BEFORE the equipment is started.
  •                    Maintain PSSR records for all projects where required (MOC’s)
  •                    Track outstanding issues from each PSSR until resolved

Mechanical Integrity–

  •                    Maintain the B-109’s (yearly inspections) and findings in APSM
  •                    Maintain the status of the open recommendation from the 5-year MI inspection
  •                    Assist the refrigeration supervisor in maintaining the PM’s for the following:
  •                    Weekly, Monthly, semi-annual, and annual PMs for all equipment to include: compressors, condensers, evaporators, heat exchangers, vessels and associated equipment
  •                    Vibration analysis of each compressor
  •                    Oil analysis
  •                    Oil consumption logs
  •                    SRV 5-year replacement schedule, due dates, etc…
  •                    Ensure a Certificate of Analysis is obtained before each Ammonia delivery and maintain same in a file

 

Hot Work Policy 

  •                    Ensure HWP’s used for work on the Ammonia system is kept with the MOC paperwork
  •                    Ensure fire extinguisher training records exist for those performing fire watch duties

Management of Change 

  •                    Maintain the MOC log for each MOC required
  •                    Complete the MOC paperwork for the site and verify that all approval signatures are on file before work is started.
  •                    Ensure the operators have received training for each change requiring an MOC and maintain records of training
  •                    Assist the refrigeration supervisor in updating the necessary information required by each MOC, to include:
  •                    P&ID revision
  •                    Equipment list update
  •                    SOP creation or revision
  •                    Ammonia inventory calculation update (corporate will assist)
  •                    Material and Energy Balance update (if needed) (corporate will assist)
  •                    All other PSI items requiring update (review PSI to determine)
  •                    PM procedures update, if needed
  •                    PHA development and tracking if required (PHA will be performed by corporate PSM manager or contractor doing the work)


 

Incident Investigation 

  •                    Act as the point contact for all Ammonia Refrigeration Incident Investigations; verify appropriate team members are involved and that the team discovers the TRUE root cause of the incident.
  •                    Maintain the Incident Investigation log for each incident
  •                    Create meeting minutes for all II team investigations and distribute minutes to team members and corporate PSM manager
  •                    Maintain odor complaint forms and file
  •                    Track status of open recommendations from the II form and distribute to team members
  •                    Ensure each II form has been signed by appropriate team members
  •                    Ensure there is a Close-Out form attached to each Incident Investigation

Emergency Response and Planning 

  •                    Maintain a list of emergency responders with safety department
  •                    Ensure Hazmat training records are maintained up-to-date for all refrigeration personnel
  •                    Work with refrigeration supervisor/safety manager to ensure PM’s are in place:
  •                    Ensure a PM exists to check Hazmat suits, regulators and equipment –monthly
  •                    Ensure a PM exists to check personal respirators – monthly
  •                    Ensure a PM exists to check the Level A suits - annually (pressure test)

Compliance Audits 

  •                    Work with the Corporate PSM Manager to verify that the CA is completed on time and includes all required personnel. Coordinate the logistics of the audit.
  •                    Participate on the 3-year compliance audit team
  •                    Maintain the tracking status of open CA recommendations
  •                    Work with refrigeration supervisor to correct audit deficiencies
  •                    Post CA recommendations for review by operators, have them sign off on review.

RMP 

  •                    Maintain up to date: PSM Team member list and Organizational Chart (Post on bulletin boards)
  •                    Verify Emergency Contact is listed correctly at all times
  •                    Verify that the Responsibilities List is up-to-date and has been reviewed with all required personnel.
  •                    Work with Corporate PSM personnel to gather information for the 5-year Hazard Assessment and RMP resubmission.
