Job Description

As the Discharge Planner, you will be responsible for planning and coordinating activities related to discharge planning of hospital patients. Developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care. You will also have therapeutic activity responsibilities in this position. Ideally, you will be a compassionate, organized, creative individual who works well within a team and has effective communication skills.

General Responsibilities:

  • Provide coordination with referral agencies regarding the pre-admission status of the patient
  • Educate patient and families on post discharge community resources available
  • Provide individual and/or group therapeutic education services
  • Lead the discharge planning process planning for post-discharge follow up to assure linage of the patient with community resources has occurred to reduce re-hospitalization
  • Connect with agency resources prior to release; connect with case manager/clinician/liaison working with persons served in the community
  • Provide weekly updates to facilities and families as needed
  • Ensure prescriptions for medications are received upon discharge; provision of resources to obtain prescriptions or medications
  • Participate in weekly multidisciplinary treatment team meetings
  • May need to assist in orientation and training of new employees
  • Record all information concisely, accurately, completely, in a timely manner, in the appropriate format, and on appropriate forms
  • Other duties as assigned


  • Current license as an LBSW or LMSW in good standing in the state of Kansas
  • Experience working with the social and behavioral needs of elderly patients
  • Experience working with community resources
  • Effective interpersonal, documentation, follow up, and communications skills are required

Working Conditions:

  • General office environment in a behavioral health hospital
  • May require standing up or walking for up to 75% of the time
  • Required to use assistance when attempting to assist, lift or carry objects over 25 lbs.
