Job Summary

The eCare Nurse Coordinator is a professional (RN) who provides ongoing oversight and coordinates care using multiple technology platforms, as well as, work with member’s primary care physician or other qualified healthcare providers to ensure the best outcome for the member. Works with members to create new behaviors with an active technology driven role in their health via the Clear Spring eCare program. By using our eCare digital tablet and platform, the eCare Coordinator will provide oversight of member’s health and assist members with health-related needs.  This role is 4 days per week / 10 hours per day.

 

Main Accountabilities:

  • Create open and trusting relationships with Clear Spring members and help them enroll in the new eCare program at Clear spring.
  • Provides a one on one patient experience by educating, supporting and intervening via telephone and video to members through the eCare Tablet.
  • Monitors members using established disease specific protocols;
  • Provides clinical support and initiatives within the scope of nursing license;
  • Develops and maintain ongoing rapport with member and caregivers via phone/or video communication;
  • Assesses and evaluates member’s biometric data; establish parameters;
  • Notifies physician and other providers of changes in member’s condition or status, updates the member’s plan of care, as needed;
  • Accept written doctor’s orders and update member’s plan of care accordingly;
  • Provides member education related to disease specific conditions.
  • Prepares reports to communicate findings to immediate supervisor;
  • Informs department manager of perceived problem areas related to patient care;
  • Demonstrates a professional attitude and sound judgment in interactions with members, physicians or any other health care providers;
  • Functions as a member of an inter-disciplinary care team, as needed;
  • Seeks consultation and direction from immediate supervisor, as appropriate;
  • Provides information about the Remote Program Monitoring (RPM) Program to members and explain the benefits of having chronic conditions managed, assessed and treated;
  • Review member’s labs, medications and preventive screening to identify member’s gap in care;
  • Performs other duties as assigned.


Qualifications:

  • Active Licensed Practical Nurse (LPN), Registered Nurse (RN) or RN BSN required
  • Preferred at minimum 3 years of Managed Care Experience
  • Minimal Nursing Experience: 1-2 years or recent acute care experience (i.e. hospital care / ER)
  • Multi-state license required;
