  • Physicians-The patient’s Attending Physician continues to provide oversight of care and Hospice Physicians consult with hospice staff and the attending physician. 
  • Nurses (RN or LPN)

  • Certified Nursing Assistants (Home Health Aides)
  • Spiritual Care (Chaplains)
  • Pharmacists
  • Trained Volunteers- offer companionship for patients and respite for families
    
    
Job Summary:
Plans, develops, and evaluates the Hospice Grief and Bereavement Program in accordance with the Hospice Conditions of Participation. Responsible for developing Hospice bereavement plans of care and for coordinating and overseeing the implementation of those plans. Responsible for all training, education and development of educational materials related to grief and bereavement in the Hospice program. Collaborate with the Hospice Volunteer Coordinators to train, supervise, evaluate and support the Hospice bereavement volunteers. Is an active member of the Hospice Leadership Team and the Hospice Interdisciplinary Team. 
 
Primary Duties:   
  1. Assesses grieving individuals/families, conducts individual and family sessions, and facilitates and conducts grief and bereavement support groups. 
  2. Develop goals and objectives in coordination of bereavement services. 
  3. Develops bereavement plan of care for each identified client based upon bereavement assessment and clinical judgment.
  4. Develops and implements annual Quality Improvement Plan for bereavement services in conjunction with overall Quality Improvement Plan for Hospice.
  5. Performs other duties as assigned. 
 
   
License, Certification, Education or Experience:   
    

REQUIRED for the position:
  • Master’s degree in Social Work, Counseling or other health-related field
  • One of the following credentials is required: Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT) or Associate License: Social Worker Associate License (LSWAA) or (LSWAIC), Associate Mental Health Counselor (AMHC) or Associate Marriage and Family Therapist (AMFT)
  •  Current Washington State driver’s license and proof of insurability
  • 2 years of experience in Social Work, Counseling or other health-related field, preferably experience with working with people who are grieving or in a hospice program
  • Knowledge of current best practices for working with bereaved adults and children in both individual and group settings
  • Provision of services in non-traditional settings including but not limited to facilities, community settings, or homes, within the boundaries of Snohomish and King Counties
  •  Maintains certification in Healthcare Provider BLS

DESIRED for the position:
  • 1 year experience in program coordination in a healthcare setting
  • Experience in program planning, development, implementation, and evaluation
