Job Summary
The In-home Mental Health (IHMH) Case Manager provides psychosocial assessment, ongoing counseling, coordinative care support, telepsychiatry support, and case management services to adult King County Medicaid recipients with both medical and behavioral health challenges that make it difficult for them to attend appointments in the community. Most services are being provided via videoconferencing and telephone support, with in-person support provided when emergent concerns develop. IHMH services are strengths-based and recovery-oriented, whereby clients are provided with the behavioral health, health care and social services needed to achieve and maintain them at the highest level of functioning attainable. IHMH Case Managers provide consultation and education to our own agency staff, to other staff at EvergreenHealth, and to the wider King County community of behavioral health professionals.
Primary Duties
  1. Provides behavioral health assessments, counseling, and case management support via either telemedicine (videoconferencing and phone calls), or at the client’s place of residence (independent home, retirement home, assisted living facility, adult family home, and nursing home).
  2. Conducts comprehensive behavioral health assessment for each client.
  3. Develops and continuously updates individualized treatment plans in collaboration with the client, and/or family, DPOA, and caregiving staff.
  4. Provides behavioral health treatment that includes counseling, case management and coordinative care support. Services are recovery-oriented, with the goal of helping the client to achieve and maintain their distinct highest level of functioning.
  5. Discharges client from services when appropriate.
  6. Performs other duties as assigned.
License, Certification, Education or Experience:

REQUIRED for the position:
  • Master’s Degree in Social Work, Counseling Psychology, Behavioral Sciences, or related field from an accredited college or university
  • Credentialed as one of the following: Licensed Mental Health Counselor (LMHC) or LMHC Associate; Licensed Independent Clinical Social Worker (LICSW) or LICSW Associate; Licensed Advanced Social Worker (LASW) or LASW Associate. Candidate must meet criteria as a designated Washington State Department of Health Mental Health Professional (MHP) in compliance with WAC 388-65-0150(1), which includes: 2 years’ experience in the direct treatment of mentally ill adults and older adults under the direct supervision of a WA State approved supervisor who is also a designated MHP.
  • If not currently licensed by date of hire, licensure as an LMHC, LICSW, LASW will be achieved within 2 years of start date.
  • Current Washington State Driver’s license and ongoing proof of insurability
  • Current Healthcare Provider BLS certification by date of hire                                                                                                                                                                   

DESIRED for the position: 

  • Licensed Mental Health Counselor (LMHC), Licensed Independent Clinical Social Worker (LICSW), or Licensed Advanced Social Worker (LASW)
  • Geriatric Mental Health Specialist (GMHS) designation in compliance with WAC 388-65-0150(2)
  • 5 years of experience providing clinical care to complex behavioral health clients, including those with borderline personality disorder, bipolar disorder, major depressive disorder, and generalized anxiety disorder
  • Adept at trauma-informed care and motivational interviewing, with knowledge of Rogerian, CBT and DBT strategies and skills
  • Deep-level knowledge of King County resources, as well as county, state and federal benefits and entitlements
  • Experience with in-home service delivery as well as virtual service delivery models
  • Experience with older adults, high-risk, medically compromised, and disabled individuals
  • Experience with EMRs, and tech-savvy
