POSITION SUMMARY:
Equitas Health Pharmacy is a subsidiary company of Equitas Health providing access, education, consultation and management of general retail and specialty pharmaceuticals for patients of Equitas Health and other Ohio providers, as well as the local community. The Pharmacy Care Coordinator of the Equitas Health Pharmacy is responsible for assisting the pharmacy team with re-engaging and maintaining patients in care. Specific patient populations include, but are not limited to: PrEP patients, patients living with HIV/AIDS, and patients receiving specialty medications. The Pharmacy Care Coordinator will often work as a liaison between the pharmacy and medical provider offices to communicate patient adherence/retention issues.
ESSENTIAL JOB FUNCTIONS:
Essential functions of the job include, but are not limited to, extensive verbal and written communication; documentation of communication in the pharmacy operating system; relationship building with patients and staff; utilizing a computer for typing and conducting research; utilizing Microsoft Excel to create tables and charts; driving or having reliable transportation.
Salary: $42,500-$53,100
MAJOR AREAS OF RESPONSIBILITIES:
· Provide excellent customer service to Equitas Health Pharmacy patients.
· Regularly contacting and communicating with patients via telephone and/or other means of correspondence.
· Collecting all pertinent patient information and providing thorough and accurate insurance verification when necessary.
· Researching and resolving insurance problems and other accessibility or adherence related issues.
· Scheduling prescription pickups or deliveries for patients.
· Work in a collaborative manner with the Equitas Health Medical Center and other Equitas Health Pharmacy employees. Be a positive representative of Equitas Health Pharmacy when interacting with patients, as well as outside vendors and clinics.
· Creating reports including, but not limited to, retention and adherence data.
· Work with physicians’ offices and insurance companies to obtain prescription authorizations and prior authorizations, when necessary.
· Routing patients requiring adherence counseling or interventions to a pharmacist.
· Understanding of Ohio HIV Drug Assistance Program (OHDAP) and 340B intricacies.
· Assisting patients with manufacturer patient assistance programs enrollment.
· Assisting clinical pharmacists with prior authorizations and prescription processing

· Other duties as assigned by the Director of Pharmacy Services.
· Any other responsibilities as defined in the Equitas Health Pharmacy Operations Manual.
EDUCATION/LICENSURE:
· Minimum of a High School Diploma/GED, Bachelor’s Degree Preferred.
· Ohio Registered or Certified Pharmacy Technician preferred.
KNOWLEDGE, SKILLS, ABILITIES AND OTHER QUALIFICATIONS:
· Two years pharmacy technician experience preferred
· Thorough understanding of pharmacy operations and laws.
· Excellent written and verbal communication skills.
· Must be exceptionally organized and able to work independently.
· Knowledge of health and social service needs of persons with HIV/AIDS.
· Must have sensitivity to, interest in and competence in cultural differences, HIV/AIDS, minority health, sexual practices, chemical dependency and a demonstrated competence in working with persons of color, and gay/lesbian/bisexual/transgender community. Proficiency in all Microsoft Office applications and other computer applications required.
· Proficiency in all Microsoft Office applications (Excel, Word, Outlook and PowerPoint), computers, email, web-based systems and electronic health records.
· Reliable transportation, driver’s license and proof of auto insurance required.
