Job Purpose: The Case Manager is primarily responsible for assisting in patient care to promote effective education, self-management support, and timely healthcare delivery to achieve optimal quality and financial outcomes. This individual is a vital member of the Case Management Team and services as an advocate for patient healthcare needs – coordinating care to minimize the fragmentation of healthcare delivery systems and thereby enhancing quality, cost-effective outcomes.

What You Will Do:

  • Providing ongoing case management to patients with identified needs, including coordinating behavioral health services, medical care, education, placement, finances, and socialization services.
  • Utilizing influencing/motivational interviewing skills to ensure maximum patient engagement and promote lifestyle/behavior changes to achieve the optimum level of health.
  • Identifying and effectively utilizing community resources to facilitate patient access to necessary services.
  • Collaborating with recipients and their families to implement effective service plans, explaining available clinical options, and coordinating care with the clinical team and other healthcare providers.
  • Maintaining accurate and timely documentation using electronic medical records (EMR) and other healthcare databases.
  • Assisting patients with overcoming barriers to attending scheduled appointments, ensuring they can access necessary healthcare services.
  • Familiarity with Department of Human Services (DHS) regulations and procedures to effectively navigate and coordinate services for patients in need.
  • Other duties assigned.

What We Need From You:

  • A bachelor's degree in counseling, psychology, social work, or a related field is preferred.
  • Qualified as a Behavioral Health Technician.
  • Exceptional communication and time management skills.
  • Exceptional problem-solving skills and the ability to work within an interdisciplinary team.
  • CPR/First Aid certification and a valid Arizona fingerprint clearance.
  • You have demonstrated the ability to work independently.
  • Work within an EMR system, Advance MD preferred
  • This position operates under a pay-for-performance (P4P) compensation structure. Candidates must be comfortable with this unique payment model.

Your Work Schedule:

  • Embrace flexibility with our hybrid model: 4 days of impactful virtual engagement and 1 day of meaningful onsite connection.
  • Monday to Friday: 8 AM - 5 PM.
