Job Purpose: As the Registered Dietician, you will play a key role on Denova’s interdisciplinary care team by providing evidence-based nutrition assessment, counseling, and care planning to support patients’ overall health and wellness. You will work collaboratively with medical, behavioral health, and allied health professionals to deliver whole-person, outcomes-driven care. This role focuses on patient education, care coordination, and individualized nutrition interventions aligned with each patient’s clinical needs, cultural preferences, and personal goals.

This is a full-time, exempt position reporting to VP of Medical Operations and supporting multiple clinic locations, with an expectation to float as needed.

What You Will Do:

  • Conduct comprehensive nutrition assessments, including review of medical history, labs, dietary intake, lifestyle factors, and social determinants of health
  • Develop, implement, and monitor individualized nutrition care plans using evidence-based practices
  • Provide one-on-one and group nutrition counseling tailored to patients’ clinical conditions and goals
  • Collaborate with physicians, advanced practice providers, behavioral health clinicians, and other care team members to support integrated care
  • Document assessments, care plans, progress notes, and outcomes accurately and timely in the electronic health record
  • Educate patients, families, and caregivers on nutrition strategies to support disease management, prevention, and wellness
  • Participate in interdisciplinary meetings, case conferences, and care coordination activities
  • Monitor patient progress and adjust nutrition interventions based on outcomes and changing needs
  • Maintain compliance with professional standards, HIPAA, organizational policies, and regulatory requirements
  • Stay current with nutrition science, clinical guidelines, and continuing education requirements
  • Contribute to program development, quality improvement initiatives, and population health efforts as needed
  • Perform additional duties as assigned to support organizational objectives.

 

What We Need From You:

  • Registered Dietician Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR)
  • Current state licensure or eligibility for licensure as required
  • Bachelor’s degree in Nutrition, Dietetics, or a related field (master’s degree preferred)
  • 1–2 years of clinical experience in a healthcare, outpatient, or integrated care setting preferred
  • Strong knowledge of medical nutrition therapy and evidence-based nutrition guidelines
  • Experience working within interdisciplinary or integrated care teams preferred
  • Strong counseling, motivational interviewing, and patient education skills
  • Ability to work effectively with diverse populations and address cultural and socioeconomic factors
  • Proficiency with electronic health records and clinical documentation
  • Excellent communication, organizational, and time-management skills
  • Commitment to ongoing professional development and continuing education
  • Valid Fingerprint Clearance Card (or ability to obtain)
  • Strong understanding of HIPAA, privacy regulations, and compliance best practices.

Your Work Schedule:

  • Full-Time: Monday to Friday, 8:00 AM – 5:00 PM
  •  Work Setting: Hybrid, with an expectation to float across clinic locations as needed
