Clinical Assessment Provider - SED/SMI
Job Purpose:
The Clinical Assessment Provider meets with new, returning, or prospective patients, asks a series of clinical assessment questions to compile an integrated whole-person behavioral health assessment, then uses this data to determine the best next course of action for that patient.
Main Duties and Responsibilities:
- Associate level clinician for SED/SMI evaluations
- Interviews patients courteously and professionally to obtain pertinent clinical information.
- Uses the DSM-5 to compile and identify patients’ clinical diagnoses.
- Completes screening of patients’ needs and symptoms to determine the appropriateness of outpatient behavioral health services.
- Assesses risk for all patients and provides safety/crisis planning and resources to patients who identify danger to self or others.
- Processes and completes documentation for patients efficiently and by protocols.
- Outreaches patients who do not attend scheduled appointments and attempt to reschedule within 24 hours.
- Other duties assigned.
Requirements:
- Bachelor’s or Master’s degree required in a related field of study. (Bachelors in nursing with at least 1 year of behavioral health experience or a psychiatric-mental health certification or a Master’s degree in Counseling, Social Work or Marriage and Family Therapy)
- Experience using the DSM-5 for behavioral health diagnoses..
- Counseling training and experience a plus, licensed by the Arizona Board of Behavioral Health Examiners or Nursing board required.
- Experience with assessment of safety and crisis processes in the state of Arizona preferred.
- Telehealth experience a plus.
- Exceptional communication skills and ability to de-escalate with the use of discretion required.
- Accurate data entry skills into an electronic medical record and an ability to multi-task multiple priorities simultaneously.
