Licensed Independent Therapist - In-Home Assessments (1099 Contract)

Job Purpose: Join Denova Collaborative Health's dedicated team in partnership with Mercy Care, offering an exceptional opportunity for Licensed Independent Therapists to earn additional income on a flexible schedule. Starting this October, this role involves conducting in-home behavioral health assessments to enhance the well-being of Mercy Care Advantage members.
Reporting Line: This role will report to the VP of Medical Operations and Behavioral Health
What You Will Do:
- Make initial contact with Mercy Care Advantage members by phone to set up appointments for in-home behavioral health assessments.
- Conduct no-charge preliminary behavioral health screenings, followed by comprehensive in-home assessments if initial screening responses indicate a need.
- Work closely with members' Primary Care Providers to integrate care and share assessment results with permission, significantly impacting patient outcomes.
- Engage in additional duties and special projects as assigned.
What We Need from You:
- Current independent licensure as an LCSW, LPC, LMFT, or PSY.D.
- Malpractice and liability insurance.
- Valid Fingerprint Clearance Card.
- Reliable transportation for in-home visits.
Your Work Schedule:
- Flexible work hours: The main hours are from 9 AM to 5 PM, Monday to Friday, with the possibility of part-time flexibility. The minimum commitment is 20 hours per week, and part-time shifts may also include a weekend day between 11 AM and 5 PM. Please note that 10-hour shifts are not available.
