Essential Functions

Quality of Care

  • Ensures quality nursing and wellness care to all residents of Independent and Assisted living & Memory Care
  • Supports the SLV Mission and practices the philosophy, nursing objectives and standards of the Department of Social Services
  • Exhibit behavior as set forth in the ESC Core Performance Standards (see attached)
  • Assist residents in the activities of daily living and medications according to individual care plans
  • Maintain safe resident living areas by changing and making beds, cleaning of bedside equipment, cleaning laundry and removal of clutter
  • Encourage residents to participate in activities offered throughout the day
  • Is a mandatory reporter for all suspected abuse situations
  • Complies with all HIPAA standards of practice
  • Other duties as assigned by Director of OP/AL.


  • Record all information pertaining to the daily care of residents in the medical record
  • Report any physical or behavioral changes in the resident to DOS or SNF charge nurse
  • Report any medication errors to Director of RHS/AL or SNF charge nurse
  • Keep an activity log of the Assisted Living unit and use for shift to shift reporting
  • Maintain consistent attendance and punctuality; follow SLV policy and procedure for time off requests and sick call
  • Able to demonstrate ability to work cooperatively with co-workers, other staff and supervisors

Human Resource Development

  • Maintain active CNA certificate (if applicable) and participate in in-service trainings provided by SLV
  • Participate in all staff meetings




  • Reports all accidents and unusual occurrences immediately to Director of OP/AL or SNF charge nurse
  • Follow all established safety procedures and precautions when operating equipment and report all unsafe/hazardous conditions to Director of OP/AL.
  • Observe and report any needed repairs or service for building or equipment.
  • Participates in Facility Emergency Plan training and fire drills, and in an emergency or disaster occurs, immediately assumes the pre-assigned emergency duties.



Required Qualifications:

  1. California CNA certification
  2. Current BLS or CPR certification
  3. High school diploma or equivalent
  4. Ability to read, speak well and be understood in the English language
  5. Satisfactory completion of physical examination including TB test
  6. Fingerprint clearance from Department of Justice


Preferred elements:

  1. At least 6 months experience as a CNA in acute or geriatric care setting
  2. Basic Computer Skills


Exposure risk:

       The RA is at high risk for exposure to blood and body fluids


Physical/Sensory Requirements:  (with or without corrective devices)

  • Able to move continually throughout the facility during the work day
  • Able to cope effectively with physical and mental performance elements
  • Demonstrate emotional stability
  • Able to lift, push, pull up to 50 pounds
  • Dexterity to pinch small objects
  • Vision adequate to read medical instruments, medical record
  • Hearing adequate to communicate with peers and residents
