Care Coordinator
Description
The Care Coordinator (CC) is a team member who facilitates patient continuity of care and preventive care within the care management team. Under the clinical direction and oversight of the patient’s Primary Care Physician and Registered Nurse, the Care Coordinator navigates patients at risk by collaborating with the patient, family, physician, nurses, and other members of the healthcare team to serve needs and expedite appropriate, cost-effective care.
The CC will support the team in the care of population health patients. They will engage patients, support the care team and take an active role in improving the health of patient population’s network providers. They will partner with the care team in providing the patient with the tools necessary to make healthy lifestyle choices, adopt life-long healthy behaviors, and close gaps in care.

The CC will support the team in the care of population health patients. They will partner with the care team in providing the patient with the tools necessary to make healthy lifestyle choices, adopt life-long healthy behaviors, and close gaps in care.
Duties and Responsibilities
The duties include, but are not limited to:
- Support individualized patient care plans
- Address patient risk factors and/or obstacles to care
- Facilitate engagement between PCP and patient
- Demonstrated ability to influence patients while motivating them to change
- Participation in required care coordination conferences
- Maintain predetermined patient caseload
- Exercise sound judgment, seek advice and sign off from RN as appropriate
- Take initiative in finding solutions to difficult and/or sensitive problems
- Participate in quality gap closures
- Work as patient advocate
- Performs additional duties as assigned
Qualifications or Education, Training and Experience
- High school or GED diploma
- Customer service or related experience in a complex environment, healthcare customer service experience preferred
- Two (2)+ years of work experience in one or more of the following settings: Acute Care, Ambulatory, Specialty, Physician Office, Care Management, Home Health Care or related
- Previous remote work experience with Electronic Medical Record documentation
- Previous Managed Care or Care Management/Care Coordination experience preferred
- Must have valid driver license
Knowledge and Skills/Expected Competencies
- Candidate must be able to follow workflows and work together with a multi-disciplinary care team within a variety of clinical and non-clinical settings
- Multitasking and prioritization working in a high-volume environment with some supervision
- Ability to undertake a variety of diverse care tasks and work collaboratively in a multidisciplinary team
- Understanding of HIPAA and Confidentiality requirements
- Strong knowledge of available provider- and patient-facing programs and Quality Metrics
- Excellent interpersonal and organizational skills
- Excellent time management skills
- Compassionate and able to handle stress
- Timely and accurate documentation of day to day activities in designated technology platform
- Adaptable to new technologies and software
- Proficiency in EMR system(s), Outlook and data entry experience preferred
