We’re seeking a Primary Care Provider to join our close-knit healthcare team in our charming rural community in Lakeside, AZ. If you are searching for a community with lakes and streams for fishing, with fresh pine air, Lakeside is the place for you.
As an integral part of our community, you will provide essential primary care services to patients of all ages, offering a wide range of health services, including preventive care, chronic disease management, and more. This is a full-time or part-time position. Relocation assistance is provided!
- Completes initial physical health assessments, progress notes, annual wellness exams, acute illness/minor injury evaluations, residential program health statements, chronic disease management , develops differential diagnostic assessments, risk assessments, patient education, progress toward treatment goals.
- Documents member’s progress toward medical health goals in the clinical record in addition to medication flow sheets according to established policy. Provides medication education and informed consent when possible.
- Documents all activities in the clinical record according to policy using approved forms
- Orders follow-up blood levels and supportive laboratory tests according to standard medical practice
- Reviews the health assessment completed by nursing staff, obtains a past and current medical history.
- Initiates contact with psychiatric provider, emergency department clinicians, and medical referral specialists according to established guidelines.
- Assesses members needs and guides the clinical team while managing treatment plan updates, in addition to initiating contact with outside Providers according to established guidelines.
- Assures positive member outcomes by providing exceptional member care and timely documentation of services by consistently meeting optimal engagement and performance standards.
- To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department.
Minimum Qualifications:
For Nurse Practitioners:
- Requires the completion of an accredited Nurse Practitioner program and FNP certification.
- Licensed by the State of Arizona Nursing Board and valid DEA certification required.
For Physician Assistants:
- Graduation from an accredited Physician Assistant Program
- Certification by the National Commission on the Certification of Physician Assistants (NCCPA)
Additional Qualifications:
- Minimum of one (1) year of providing primary care experience as a medical professional.
- Experience working in an integrated behavioral health setting highly preferred.
- Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
- TB Test clearance is required at the time of hire.
- Strong commitment to rural healthcare and community well-being.
- Excellent interpersonal and communication skills.
- Strong computer skills and typing, i.e., EHR/EMR systems and MS Office and ability to learn new systems as applicable.
- Demonstrated ability to remain calm and use the proper de-escalation methods in crisis situations.
