Registered Nurse (Part Time)

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.
We are seeking a Part Time Registered Nurse (RN) in Mercer, N.J. The Registered Nurse delivers care to individuals with disabilities in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.
Hourly Pay Rate: $38.00
Responsibilities
- Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
- Create and distribute medical/health reports detailing general health, health care goals, and
physician recommendations - Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an
interdisciplinary team to include any concerns, observations, and behavioral developments - Train employees on the medical needs of assigned individual
- Monitor health and medical conditions of individuals and refer for medical services as needed
- Monitor the use of medications through medication administration logs
- Ensure program documentation and billable records are completed accurately and timely
- Participate with individuals in community, social, and recreational activities
- Participate in circles of support for personal futures planning
Minimum Requirements
- Valid R.N. license
- Valid driver’s license with a satisfactory driving record
- Ability to calculate figures related to medication dosages
- Ability to effectively communicate with program participants, third-party agencies, staff, and members of the community
