For 80 years, we have been one of upstate New York’s largest providers of programs and services for individuals who have disabilities. Many of the innovative programs and vital services that we offer are not available elsewhere.     

Position:  Service Coordinator

Location/Schedule:  Hybrid Schedule (1 day per week in office located in Albany, NY).  This position Monday through Friday and offers flexible, day time scheduling. 

We are searching for a Service Coordinator to join our agency.  

The Service Coordinator will be responsible for the delivery of Service Coordination duties and responsibilities while maintaining regulatory compliance to individuals living in the community who have a traumatic brain injury (TBI) or who are enrolled in the Nursing Home Transition and Diversion waiver (NHTD).    

Responsibilities:      

  • Establishing and maintaining regular contact with individuals we support and advocating on their behalf
  • Developing and updating services plans with strict deadlines
  • Linking individuals and applying for needed services
  • Assisting with maintaining benefits and collaborating with other services providers

 

Compensation for this position is $21.98-$24.67 per hour commensurate with experience. 

Requirements:     

  • Associate’s Degree required, Bachelor’s Degree is preferred
  • At least 2 years of experience providing case management/service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors
  • 1 year of experience providing case management/service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors will be accepted along with a Bachelor’s Degree
  • Ability to handle multiple tasks simultaneously, effectively communicate, make decisions and problem solve
  • Must be familiar with Microsoft Word and Excel
  • Reliable transportation to travel to locations throughout the greater Capital Region area
