Intake Specialist

The Center for Elders’ Independence is a PACE (Program of All-Inclusive Care for the elderly) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly, including an Adult Day Health Center, and promoting autonomy, quality of life and the ability of individuals to live in their communities.

The Position: The Intake Specialist oversees the intake and enrollment process of new PACE participants. This employee must work as part of a team along with Marketing Account Manager (AM) to educate new potential participants and their caregivers on the benefits of CEI’s PACE Program. They will help move participants through the LOC (level of care) process and be responsible for enrolling them.  They are located at the PACE Center and will coordinate with the IDT at their centers and benefits and marketing departments. The Intake Specialist will work independently to gather information, set appointments for assessments and document skilled needs for the incoming cases.  They are responsible for understanding many medical and social indications of skilled nursing home level of care and documentation of those indications in salesforce. They will work with family caregivers and participants to tour them at the PACE centers and to “close the sale” by getting the enrollment agreement signed.

DUTIES AND RESPONSIBILITIES:

  • Oversee the intake and enrollment process of new PACE participants
  • Work as part of a team along with Marketing Account Manager (AM) to educate new potential participants and their caregivers on the benefits of CEI’s PACE Program
  • Tour participants and family caregivers at the PACE centers and help sell the services.
  • Help move participants through the LOC process and be responsible for enrolling them.
  • Present all approved cases that are enrolling to the IDT at their centers.
  • Understand and gather medical and social information that leads to establishing nursing home level of care and document that information and the enrollment steps in Salesforce.

 

QUALIFICATIONS:

  • Bachelor’s degree required in related field. 
  • Experience in social work and health care where they have conducted psychosocial assessments at least a year.
  • Minimum one year experience working with elderly and their families preferably in a healthcare or community agency setting required. 
  • Since they will have direct responsibility for discussing and explaining the patients’ rights prior to enrollment in the Program of All-Inclusive Care for the   Elderly (PACE) and be required by the State of California to be a certified  expert on the PACE rules and regulations, all candidates must  be above reproach and maintain a spotless background check.
  • Must have good communication and presentation skills, both oral and written in English and the bilingual language. 
  • Marketing/Sales experience is preferred.
  • Bilingual: Spanish/English is preferred.
  • Needs to have valid CA Driver’s license and have the ability to conduct home visits and travel from center to center.   
  • Must be proficient with computer Microsoft Office suite.

DESIRED QUALIFICATIONS:

  • Strong ability to convince, persuade, or “close the sale”.
  • Must be a strong team player and able to keep a team coordinated and briefed.
  • Strong technology/EMR/Salesforce documentation experience a plus.
