Catholic Charities of Orange, Sullivan & Ulster, is looking for a full-time Case Manager in their Ulster County Case Management program.  
The hourly rate for the position is $23.00/hour.  This position comes with benefits.
Summary: This position provides direct case management services to the needy and disenfranchised in Ulster County. It is expected that this is accomplished through advocacy, providing information, referral, crisis intervention services, and short-term case management. The position will also have a role in the reception area, answering telephones and processing clients for services.

Essential Duties and Responsibilities include the following:
1. Provide crisis intervention to the homeless individuals and families facing housing emergencies.
2.  Serve as a primary case manager for the New York State Rental Supplement Program Plan (RSP), Shelter Arrears Eviction Forestallment program (SAEF), and emergency relief program.
3. Conducts a thorough needs assessment and develops a service plan to address identified, concrete needs using a short-term intervention model.
4.  Conduct social care screenings of Medicaid members using the standardized Accountable Health Communities (AHC) Health-Related Social Needs Screening tool on the Unite Us platform and refer 
case to the Social Care Manager as needed.
5. Navigate individuals who are not eligible for enhanced services to the non-Social Care Network 
services (Existing local, state, and federal programs and services as appropriate.
6. Thoroughly log all client interactions in Unite Us, Apricot, and/or other systems as required 
for the program.
7. Keep accurate and complete work files, including records, documentation, and other materials.
8. Maintains current and accurate documentation of all activities and services provided to clients.
9. Process cases in a reasonable timeframe, preferably 45 days.
10. Answers telephone and provides information as needed.
11. Conduct outreach to different communities and perform data collection activities.
12. Assist with food/kids’ pantry distributions.
13. Participate in outreach activities on the weekend and after hours, such as community tabling 
events, community outreach, and pop-up pantry. In addition, participate in special projects. These 
include Stuff- The-Bus (June and November. Adopt-A-Family and Giving Tree (Christmas), Back to 
School supply distributions (August).
14. Develops and maintains working relationships with providers throughout Ulster County.
15. Attend community or service provider meetings.
16. Available for planned activities on the weekend and some after-hours.
17. Participates in regular supervisory sessions with the immediate supervisor.
18. Attends the Agency’s in-service training.
19. Travel to meet clients at other CCOSU offices.
20. Other duties as assigned by the supervisor or demands of the program.
Position Type and Expected Hours of Work:
This is a full-time position. Days and hours of work are generally 9:00 AM to 5 PM Monday – Friday. 
Additional hours may be required to meet program deadlines or client needs.

•  Ability to work with the public and/or under potentially stressful conditions.
•  Basic understanding of client engagement and de-escalation techniques.
•  Prepare comprehensive progress notes and service plans.
•  Remain stationary at a workstation and use a computer at least 30% of the time.
Qualifications
Education and/or experience required:
•  Bachelor’s degree in Social Work or related field – Preferred
• Associate's degree with one year of experience in Human Services
•  Must possess a clean NYS Driver’s License and maintain it clean throughout employment
Skills, Licenses, and/or competencies required:
•  Knowledge of Social Services available in Ulster County – Preferred.
•  Computer literate with knowledge of Microsoft Word, Excel, and Outlook.
•  Excellent interpersonal and communication skills.
•  Good organizational, time-management, and communication skills.
• Individual must possess the ability to work well independently as well as part of a 
team.
