Position Title:            Mental Health Case Manager

Division:                    Family & Children Services of Silicon Valley

Department:              Medi-Cal Mental Health

Reports To:               Mental Health Clinical Manager

Classification:           Non- Exempt, Hourly

Salary Range:            $28+ DOEE

Position Summary:   The position of Mental Health Case Manager is under the supervision of the Clinical Supervisor and Clinical Manager, with the requirement that 60% of time at minimum, is Medi-Cal billable or services being provided to clients. This position must be available to respond to Mental Health intakes as needed for adults with serious mental health disorders.

Essential Duties and Responsibilities include the following. Other duties may be assigned. 

         Provide direct case management and rehabilitation services to adults/youth using

Wellness and Recovery model of care per the Santa Clara County Mental Health Department system of care. Most clients have dual diagnoses, both mental health and substance abuse disorders. 

         Assess, consult regarding diagnosis; use engagement skills to support the development narrative summaries and treatment plans using Transformational Care Planning model for all clients assigned to caseload in collaboration with the assigned MH therapist. 

         Plan and administer case management and mental health rehabilitation services to assist clients in accessing resources and reduce functional impairment due to mental health disorders and related problems. 

         Identify changing needs of clients and assist clients to transition appropriately to higher or lower level of care. 

         Discuss progress toward treatment goals with client. 

         Consult with psychiatrist and/or other specialist concerning treatment plan, and update treatment plan as directed. 

         Refer clients to supportive services to supplement treatment. 

         Provide services at Family and Children Services location, as well as in client homes and in the community (doctors’ offices, Social Security Administration, court, etc.). 

         At times transport clients to access services. 

         Maintain excellent client records and complete all agency and/or third-party payer required reports and paperwork. 

         Comply thoroughly with County Mental Health contract documentation and auditing standards and requirements in a timely manner. 

         Participate in supervision and team meetings.

         Education – Bachelor’s degree in social work, psychology, human services or related field required. 

         Experience – Two years direct service experience required. 

         Cultural competencies strongly emphasized: ideal applicants will demonstrate professional and/or personal experience with a variety of racial, cultural, linguistic, sexual identity, disability, and recovery populations. 

         Bilingual Spanish and English strongly preferred. 

         Lived in community, educational and/or professional experience with diverse LGBTQ culture and issues preferred 

         Prefer knowledge of Medi-Cal Mental Health policies, procedures and requirements. 

         Must have and maintain current California driver’s license and functioning automobile. 

         Computer skills - Proficient using Word, Excel, Outlook, and Internet Explorer. Electronic Health Record knowledge and experience desirable.

While performing the duties of this Job, the employee is regularly required to: 

         Communicate with clients and co-workers to exchange accurate information. 

         Frequently required to remain in a stationary position for most of the workday. 

         Move about the office to access file cabinets, office machinery, etc. 

         Constantly operates a computer and other office machinery, such as copy machine, printer, fax. 

         Must be able to review client charts for accuracy and information. 

         Constantly changing body position during play therapy sessions. 

         Constantly going to different locations in order to manage client’s needs. 

         Occasionally required to move equipment weighing about 15 pounds during play therapy sessions or moving files. 

         Occasionally works outdoors and will move to different locations regularly. 

Special Requirements:

         Must be able to meet and receive a criminal records clearance, as required by Title XXII, other licensing regulations, and Caminar practices.

         This position requires frequent driving. A valid California driver license, reliable personal vehicle, current personal auto insurance as required by law, and an MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies are all essential job requirements. 

         Must be able to pass post offer, pre-employment medical and drug tests as required under State Community Care Licensing regulations and/or agency policies.

         May be required to obtain and maintain First Aid and CPR certification.
