Position Description:    Under supervision of the Program Director, the Family Partner (FP) functions as an interdisciplinary team member, assisting the family members of consumers as well as the medical, case management and other support staff.  FP’s provide strength-based advocacy, family support services and opportunities for empowerment for family members of those with a SMI. The FP must have personal, lived experience with a family member with a mental illness and must be willing to be identified accordingly.  Because of their life experience in caring for and assisting family members with SMI and navigating the systems of care, FP’s are able to use their unique experience to inspire hope and provide support to others who are facing similar challenges with their own family members.  An essential objective of this position is to provide insight and perspective that can only be gleaned from personal experience with a family member with a mental illness.

Essential Duties & Responsibilities:

  • Serve as a liaison among family members, the treatment team and other community support providers; provide support and resource information to both family members of consumers as well as their interdisciplinary treatment team.
  • Participate and contribute to Caminar Family Support Groups under direction of the Program Director.
  • Assess the needs of families, facilitate referrals, exchange information, and resolve issues and concerns.
  • Schedule and conduct home visits with assigned families in coordination with staff and outside agencies.
  • Participate in and encourage family involvement in services, activities supporting community awareness, parent trainings, and events.
  • Function as a role model, exhibiting competency in self-care and use of coping skills.
  • Provide information and support to mental health consumers in outpatient and inpatient settings. Serves as a consumer advocate.
  • Assist individuals with mental illness or co-occurring disorders to achieve their personal recovery goals by promoting self-determination, personal responsibility, and the empowerment inherent in self-directed recovery.
  • Assist with and teach activities of daily living, such as meal planning and preparation, personal hygiene, budgeting, attending scheduled care appointments, assistance with paperwork and benefits acquisition, employment and/or job search assistance, interpersonal and socialization skill development, recreational and volunteer activities, and WRAP assistance.
  • Support and assist with maintenance of personal health, including attainment of and follow through with medical, psychiatric, and dental care.
  • Develop and maintain a respectful, caring, tolerant, ethical, and empowering relationship with all individuals served while maintaining professional boundaries.
  • Assist consumers in connecting with and maintaining relationships with outside-the-system resources in order to increase independence and more actively participate in their communities.
  • Attend and participate in all team meetings as scheduled and sharing information regarding all important interventions.
  • Report any “at risk,” unusual, or illegal activity by clients to appropriate members of the team (i.e. Warm line Counselor, Case Manager, medical staff or Emergency On-call person) in a timely manner.
  • Work collaboratively with consumers and the treatment team to provide appropriate and culturally sensitive delivery of services.
  • Drive personal or agency vehicle to treatment destinations; documenting and reporting mileage according to agency procedures so that services can be provided in a timely manner; comply with agency vehicle/driver policies and procedures at all times.
  • Assist in monitoring cleanliness and maintenance of client living environments, providing assistance as necessary in order to meet appropriate standards.
  • Assist in resolution of conflicts between and among roommates and housemates.
  • Maintain and follow established procedures for safeguarding and delivering client allowances and gas cards. Provide all receipts to assigned administrative staff in a timely manner.
  • Attend and/or timely complete on-line trainings as required.
  • Participate in promoting a safe, healthy and clean working environment at all times consistent with applicable laws, industry standards and the agency’s own Health & Safety Program.
  • Complete and submit accurate time sheets and absence reports to program management in a timely manner.
  • Promote within the agency and with the general public the philosophy and practice of social rehabilitation.

  • Perform other related duties, responsibilities and special projects as assigned.

Experience, Qualifications, Skills & Abilities:

  • Requires an understanding, appreciation, and unwavering commitment to the philosophy and mission of Caminar.
  • An essential requirement of this position is to provide insight and perspective that can only be gleaned from personal experience with a family member with a mental illness.
  • A high school diploma or GED required. One (1) year of experience providing peer support, advocacy, community outreach or related support services or two (2) years of experience working in a behavioral health and/or criminal justice service setting preferred.
  • Must be available to work morning, evening and weekend shifts on a short notice and to work overtime when required. Ability to keep a flexible work schedule is therefore essential.

Special Requirements:

  • Must be able to meet and receive a criminal records clearance, as required by Title XXII, other licensing regulations, and Caminar practices.
  • This position requires frequent driving. A valid California driver license, reliable personal vehicle, current personal auto insurance as required by law, and an MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies are all essential job requirements.
  • Must be able to pass post offer, pre-employment medical and drug tests as required under State Community Care Licensing regulations and/or agency policies.
  • A personal cell phone with reliable service and, if applicable, a data plan to use for business purposes.
  • May be required to obtain and maintain First Aid and CPR certification.
