Position Description:  The position of Case Manager is under the supervision of the Program Director, with the requirement to maintain an active caseload of 35-40 clients.  This position must be available to respond to clients being released from the jail and the Mental Health Collaborative Court. Caseloads will consist of adults who are experiencing mental health and co-occurring substance abuse disorders. The clinical team works under the clinical supervision of a highly experienced clinician, with robust operational support.

 

Essential Duties & Responsibilities:
  • Working with clients to assist them to succeed in their re-entry plans.
  • Provide direct service to adults using strengths based case management.
  • Meeting with clients at the jail to engage with them pre-release.
  • Attend Mental Health Collaborative court appearances with clients.
  • Identify changing needs of clients and assist clients to transition appropriately to higher or lower level of care.
  • Discuss progress toward treatment goals with client and collaborative court.
  • Consults with medical doctor or other specialists concerning treatment plan and amends plan as directed.
  • Communicate regularly with the Court staff and community linkages.
  • Assist clients in getting immediate needs met when where released from jail-housing, medical benefits, ID cards etc.
  • Provide case management linkages clients and families to community resources.
  • Comply thoroughly with County Mental Health contract documentation and auditing standards and requirements in a timely manner.
  • Participate in group and individual supervision.
  • Perform other related duties, responsibilities and special projects as assigned.

 

Qualifications, Skills & Abilities:

  • Must be passionate about Caminar's mission.
  • Bachelor’s degree in mental health/social work or closely related field.  Master’s degree preferred.
  • Bilingual English/Spanish preferred.
  • Thorough clinical knowledge and experience in providing mental health service to dually diagnosed adults.
  • Thorough understanding and experience in the delivery of services to an ethnically and economically diverse population.
  • Medi-Cal Mental Health experience in Solano County preferred.
  • Excellent computer skills in a Microsoft Windows environment including email and Internet navigation required. Experience with Human Service Information Systems strongly preferred. Experience with web based time and attendance and staff training and development system strongly preferred.

 

Special Requirements:

  • Must be able to meet and receive a criminal records clearance, as required by Title XXII, other licensing regulations, and Caminar practices.
  • This position requires frequent driving.  A valid California driver license, reliable personal vehicle, current personal auto insurance as required by law, and an MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies are all essential job requirements. 
  • Must be able to pass post offer, pre-employment medical and drug tests as required under State Community Care Licensing regulations and/or agency policies.
  • A personal cell phone with reliable service and, if applicable, a data plan to use for business purposes. 
  • May be required to obtain and maintain First Aid and CPR certification.
