Job Summary & Responsibilities





Job Summary

This role provides clinical care including primary care, continuity care, acute care, co-management of patients with healthcare team, and participates in coverage of providers at Brown & Toland Health (“BTHealth”). This position will work closely with the clinical team and is primarily responsible for managing direct patient care with the implementation of evidence-based medicine and adherence to national and internal practice guidelines and policies. This position will also play a key role at driving clinical initiatives for BTHealth.



Essential Functions

  • Provides high quality care to patients in collaboration with the clinical team and BTHealth leadership.
  • Provides direct and indirect patient care as part of the treatment regime, identifying actual or potential risks based on patient screening data.
  • May serve as the primary provider of care for patients as appropriate and based on scope of practice.
  • Collaborates with providers, ancillary personnel, community resources and other interdisciplinary team members to proactively resolve patient needs.
  • Leverages the use of the Electronic Health Record (“EHR”) to manage and improve patient care, workflows and clinical compliance.

  • Bachelor’s degree or equivalent from 4 year university, preferably in biology, chemistry, physiology, psychology or related field of study
  • Degree and certification from accredited RN program

  • Minimum of 1-2 years clinical experience.
  • Recent graduates are also encouraged to apply.

Preferred Experience

  • 5+ years professional experience working as a Nurse Practioner
  • Significant experience working with an EHR, preferably Epic




Required Certifications

  • Active and in good standing RN licensure and Nurse Practioner Certification in the state of California
  • Active and good standing NPI number
  • Active and good standing DEA number from US Drug Enforcement
