Summary
With a passion for care towards residents, families and team members, the Lifestyle Coordinator (Recreation) is accountable for collaborating with the Interdisciplinary Team and residents in promoting an interdisciplinary team approach to resident care, aiding and assisting with service excellence, program development, implementation, coordination and ongoing evaluation.
Essential Duties, Responsibilities, and Performance Measures
Resident Centered Care: Promotes service excellence through planning, organizing and implementing resident focused recreation activities for the Young Adults program and by collaborating with the Interdisciplinary Team and community agencies which provide additional care, support, and service.
Performance Measures include:
- Respects the resident’s rights and individuality.
- Promotes and is responsive to resident’s holistic needs.
- Provides service as indicated by Recreation Therapy assessment and resident goals.
- Informs the Therapist when resident needs alter and assists in implementing change.
- Provides functional support and assistance in the area of recreation activities, mobility, comfort, self-care and intellectual stimulation under the direction of the Occupational Therapist and Recreation Therapist.
- Works collaboratively with the resident while maintaining an understanding of the resident’s need to make choices consistent with their own values and preferences.
Documentation: Accurate, comprehensive, and timely documentation of resident data.
Performance Measures include:
- Documents in health care records, including resident centered care plans as required.
- Documents summary of resident’s progress, involvement in groups, and 1:1 programs as required.
- Communicates resident changes and unusual occurrences as appropriate.
Safety: Contributes to a safe environment for residents, the team and self by understanding and adhering to safety guidelines and practices at all times.
Pro-active Learner: Demonstrates the vision and values of Bethany and lives out the mission of Creating Caring Communities.
Performance Measures include:

- Demonstrates a passion for care towards residents, families and team members.

Performance Measures include:
- Fosters collaborative relationships with families and community groups to ensure the highest quality of life for residents.
- Liaises with the community and community resources to ensure adequate recreational opportunities and recreation support for residents.
- Promotes constructive caring relationships with the corporate, facility and interdisciplinary support teams, as well as residents, families, volunteers, community resources and visitors.
- Assists the Interdisciplinary Team to provide resident recreational centered services.
- Assists in the review of recreational programs and actively participates in the implementation and evaluation of new therapeutic recreational programs and services.
- Contributes to the establishment of short and long term goals for the resident.
- Assists in coordinating and guiding volunteers who participate with therapy programs and orientates them to programs.
What You Bring
- Grade 12 Diploma or equivalent.
- Community Rehabilitation or Therapy Assistant Diploma or equivalent preferred.
- Class 4 Driver’s License.
- 2 years’ experience in long term care or health related field.
- 2 years' experience in therapeutic recreation.
- Able to work effectively in a physically and mentally demanding environment.
- Excellent interpersonal skills and strong verbal and written communication skills.
- Experience working with and coordinating volunteers.
- Working knowledge of Microsoft Office.
