Full Time Mental Health Case Manager

Position Overview:
Mahajan Therapeutics is looking for a hard-working and conscientious Case Manager to perform various administrative tasks with a keen eye for detail. Works under the supervision of the Case Manager Supervisor and Clinical Director. The Case Manager’s primary job function is to enhance the overall treatment process by assisting therapists to meet the overall treatment needs of clients. Performs other duties as assigned.
Responsibilities:
• Act as a liaison between therapist, the referring agency, and family of the client. Arrange family visits, coordinate transportation, and ensure clients’ needs are met.
• Ensure communication between family, referral source and Mahajan therapeutics is agreeable and ongoing.
• Attend court hearings as deemed appropriate.
• Ensure documentation occurs according to policy.
• Attend treatment staffing with prepared report that accurately represents client’s response to treatment.
• Facilitate individual and group Case management as assigned.
• Function on agency quality assurance or other committees as assigned.
• Assist with in-service training as directed by Clinical Director or CM Supervisor.
• Attend supervisory sessions with prepared agenda.
• Ongoing assessment of client needs
• Assistance in achieving personal independence in managing basis needs as identified by the client and/or parent or guardian
• Facilitation of further development of daily living skills, if identified by the client and/or parent or guardian
• Coordination of the ITP, including:
a) services identified in the ITP
b) assistance with accessing natural support systems in the community
c) linkages to formal community services and systems
• System monitoring
• Coordination and assistance in crisis management and stabilization
• Advocacy and outreach
• As appropriate to the care of clients, education and training specific to the individual needs, abilities and readiness to learn
• AOD interventions that address symptoms in relation to their drug or alcohol use, abuse, and dependency. Interventions assist a client in eliminating barriers to seeking or maintaining sobriety.
• Activities that increase the client’s capacity to positively impact the client’s environment
• Adequate knowledge and ability to navigate EHR (Electronic Health Record) system.
• Assume additional duties as assigned by Clinical Director or CM Supervisor.
Qualifications:
• Education: Bachelor’s degree in counseling, related field, or minimum of two years college credit plus five years of work experience in the field of Addiction or CDCA requirements adequate to perform job duties according to Ohio Law.
• Required Licensure: CDCA / LCDC II / LCDC III
•Preferred Experience: Experience working with adolescents, teens and adults with an AOD diagnosis, or AOD work history.
• Must demonstrate a positive, non-judgmental, and accepting attitude toward our patient population.
• Satisfactory criminal background check and drug screen.
