Title: Director – Patient Care Services  
Department – Hospice  
 
About the Role:
The Director of Patient Care Services will oversee and guide the clinical operations and strategic direction for the lines of businesses pertaining to Hospice within Astrana Health. This nurse leader will play a critical role in strategizing and implementing ways to optimize quality of care for terminally ill patients, providing leadership to our hospice healthcare teams, and scaling our hospice department. The ideal candidate will have extensive experience in end of life care, a deep understanding of clinical and administrative operations pertaining to hospice, and a proven track record of leadership in a healthcare setting.
 
What You’ll Do:
  • Oversee the provision of hospice care services to ensure patient safety, comfort, and quality of life.
  • Collaborate with physicians, nurses, and interdisciplinary teams to develop, implement, and evaluate care plans.
  • Ensure that care meets regulatory and compliance standards, including Medicare and state-specific requirements.
  • Review and update patient care policies and procedures as necessary to align with best practices and organizational goals.
  • Develop and implement strategic initiatives to scale hospice programs.
  • Provide guidance and support for clinical staff in complex patient cases, symptom management, and end of life care.
  • Provide mentorship and foster a collaborative work environment among staff.
  • Establish quality metrics, benchmarks, and performance targets to monitor and improve clinical outcomes, patient safety, and member satisfaction. 
  • Lead the development and implementation of quality improvement initiatives aimed at enhancing patient outcomes.
  • Work cross-functionally with finance am to develop hospice budgets and ensure care services are delivered cost-effectively.
 
Qualifications: 
  • California Nursing license – RN preferred
  • Extensive hospice experience
  • Strong understanding of managed care principles, healthcare delivery systems, and regulatory requirements.
  • Proven experience in developing and implementing strategic initiatives in a healthcare setting.
  • Excellent leadership, communication, and interpersonal skills.
  • Ability to analyze complex clinical data and translate findings into actionable strategies.
  • Demonstrated ability to lead and inspire teams in a dynamic healthcare environment.
 
Preferred Qualifications: 
  • Experience with value-based care.
  • Familiarity with healthcare technology and data analytics tools.

  • Our organization follows a hybrid work structure where the expectation is to work both in office and at home on a weekly basis. The office is located at 1660 S. Garfield Avenue, Alhambra, CA 91801.
