About the role:      
 
The Social Services Coordinator is a key member of the care coordination team responsible for providing advocacy and navigation services to patients, families, and caregivers. The Social Services Coordinator works with patients to ensure that they receive the highest quality of care and have access to all available resources. This role requires strong communication and interpersonal skills as well as the ability to provide emotional support and problem-solving assistance to those in need.
 
What You’ll Do:
  • Provide advocacy and social services to patients, families, and caregivers. 
  • Establish trust and rapport with new patients, existing patients, families, and caregivers. 
  • Educate patients and families on their available resources, such as housing, utilities, and food assistance. 
  • Support patients with Social Security disability, retirement, SSI Benefits, replacement cards. 
  • Advocate for patient rights and ensure that they receive the best possible care, including referrals and coordination of care, such as appointments for specialists, referrals to mental health, Non-Emergent Medical Transportation. 
  • Assist patients with Welfare applications such as Medicaid, SNAP/EBT, etc. 
  • Support patients with Department of Motor and Vehicle and Immigration services.  
  • Develop and implement patient-centered care plans and department Standard Operating Procedures. 
  • Problem solve and follow up with patients to ensure that they are receiving the care they need. 
  • Collaborate with Care Coordinator/ other healthcare providers to ensure patient safety and quality care. 
  • Follow established policies and maintain patient confidentiality and adhere to HIPAA guidelines. 
  • Navigate through appropriate interventions such as de-escalation, Crisis Prevention, motivational interviewing, etc.
  • Complete, document, and report daily activities, telephone call, department and patient meetings, etc. 
  • Ability to manage a weekly caseload of a minimum 70 patients.
  • Complete additional tasks and duties as assigned.
 
Qualifications:
  • Bachelor’s degree in social work, healthcare, or a related field  
  • 2+ years of experience in patient advocacy or a related field  
  • Knowledge of healthcare regulations and best practices 
  • Excellent communication and interpersonal skills  
  • Ability to provide support and problem-solving assistance 
  • Strong organizational and time management skills 
  • Foster a healthy work culture and ability to work alone or in a team  
  • Capability to work in a fast-paced environment and adapt to new policies or workflows 
  • Ability to maintain patient confidentiality  
  • Proficiency in Microsoft Office (Excel, Word, Outlook) and electronic medical records
  • Up to 60% travel within clinics to meet patients for their Social Service needs
  • Bilingual, Spanish

  • Community Health Worker (CHW) Certification 
  • 4+ years previous experience

Environmental Job Requirements and Working Conditions:     
  • This position is based in one or more of our clinic locations in the Las Vegas Metropolitan Area.
  • The annual total compensation target pay range for this role is: $20/hr-$24/hr. This salary range represents our national target range for this role.
