About the Role:
We are looking for a Patient Engagement Specialist I to join a team passionate about promoting patient health. As part of the Quality Department, the Clinical Operations team coordinates and delivers key clinical care to improve health outcomes. Through engagement and education, we encourage patients to utilize preventive services and to be active in the management of chronic diseases. Together, we strive to achieve healthy living for all through all stages of life.
As a Patient Engagement Specialist I, you represent the patient’s care team and Astrana Health. You will engage with patients, communicate essential health messages, and connect them to resources. Your primary duty will be to provide health education, schedule appointments with our internal clinical care team, and encourage utilization of recommended services.  You may also be asked to carry out other duties in support of department programs and goals.
 
What You’ll Do:
  • Conduct outreach calls to inform patients of recommended services in a courteous and engaging manner
  • Promote disease prevention and chronic disease management messaging
  • Review information through internal portals to determine patient qualifications for clinical services
  • Schedule appointments for services with our internal team
  • Input patient information accurately into electronic health record system
  • Assist patients with health questionnaires as needed
  • Call patients to confirm appointments and provide preparation instructions
  • Follow up with patients who missed their appointments
  • Assist with administrative tasks for the department, including, but not limited to: calling providers regarding patient information, receiving incoming calls regarding department programs, sending faxes to providers, receiving and redirecting incoming fax, uploading files
  • If applicable, provide translation assistance in department programs
  • Uses, protects, and discloses our company’s patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.

Qualifications:
  • Associate’s Degree required
  • Preferred but not required - Bilingual Spanish or Chinese (Cantonese and/or Mandarin)
  • 1+ year experience in sales, customer service, or outbound calls
  • General understanding of preventive care services
  • Experience using Microsoft applications such as Word, Excel, and Outlook
  • Equivalent combination of education and/or experience
  • Must be able to project a professional, pleasant image while interacting with a variety of personalities and cultures
  • Must demonstrate tact and diplomacy in interpersonal interactions
  • Should have strong communication and customer service skills and respect for confidentiality
  • Must have ability to plan, prioritize, and complete tasks
 
You’re great for the role if:
  • Experience in clinical/medical settings OR working in healthcare/managed care environment, such as IPA, MSO, or health plan
  • Experience using EHR systems
  • Experience with Medicare STAR Metrics and HEDIS Measures
  • General understanding of medical conditions and specialties

Environmental Job Requirements and Working Conditions:
  • This is a remote position based in the US. The home office is located at 568 W. Garvey Ave, Monterey Park, CA 91754.
  • The total compensation target pay range for this role is: $22.00-25.00 per hour. The salary range represents our national target range for this role.
