ACM Coordinator I (Hawaii Resident)

Job Title: ACM Coordinator I
Department: Health Services - ACM
Must currently reside or have resided in Hawaii
We are currently seeking a highly motivated Case Management Coordinator. This role will report to an ACM Supervisor and enable us to continue to scale in the healthcare industry.
What You'll Do:
- Responsible for member/provider outreach and notification (telephonic and/or written) utilizing department templates
- Responsible for assisting ACM nurses with member assessment and communication, which includes but is not limited to eligibility verification, requesting medical records, creating case, and documenting care activity
- Participate in all health plan and department meetings and trainings as applicable
- Assist with health plan audit preparation
- Meet standard benchmarks with performance objectives
- Provide ongoing support and mentorship to other team members as needed.
- Answer inbound phone queue calls timely to assist members and/or providers.
- Assist in delegated projects and assignments
- Perform other duties, projects and actions as assigned
Minimum Qualifications:
- High school diploma OR GED
- Experience using Microsoft applications such as Word, Excel, and Outlook
- Experience working in healthcare setting preferred
You're great for this role if:
- Previous experience with Case Management
- Experience working in a MSO/IPA/Health Plan
- Able to speak and understand Tagalog or Ilocano (Preferred)

Environmental Job Requirements and Working Conditions:
- This position is remotely based in the U.S. The home office is located in Monterey Park, CA.
- Employees must be able to work standard business hours in Hawaii Standard Time (HST)
