Job Title: Patient Care Coordinator   

Department: Quality - Clinical Operations   

About the Role:
We are looking for a Patient Care Coordinator to join a team that is passionate about the health of our patients. As part of the Quality Department, the Clinical Operations team leads the effort to improve health outcomes by coordinating and delivering key clinical care. Through outreach and education, we inspire patients to take a preventive approach to maintaining their health and to be active in their management of chronic diseases. Together, we strive to achieve healthy living for all through all stages of life.
As a Patient Care Coordinator, you are part of an essential patient outreach team that promotes key health messages and services. Your role will be to promote recommended services, provide health education, and schedule appointments with our internal clinical care team. You may also be asked to carry out other duties in support of department programs and goals.   

What You’ll Do:
•    Conduct outreach calls to inform patients of recommended services
•    Promote disease prevention and chronic disease management messaging
•    Schedule appointments for services with our internal team
•    Call patients before appointment to remind them of date and time
•    Follow up with patients who missed their appointments
•    Assist patients with health questionnaires
•    Input patient information accurately into electronic health record system
•    Assist with administrative tasks for the department, including, but not limited to: receiving incoming calls regarding department programs, sending faxes to providers, receiving and redirecting incoming fax, uploading files
•    If applicable, provide translation assistance in department programs
•    Uses, protects, and discloses our company’s patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
•    Other duties may be assigned   

Qualifications:
•    Associate’s Degree required
•    Bilingual - Chinese (Cantonese and/or Mandarin) 
•    Prior experience in a customer service or outreach role
•    Experience using Microsoft applications such as Word, Excel, and Outlook
•    Equivalent combination of education and/or experience
•    Must be able to project a professional, pleasant image while interacting with a variety of personalities and cultures
•    Must demonstrate tact and diplomacy in interpersonal interactions
•    Should have strong communication and customer service skills and respect for confidentiality
•    Must have ability to plan, prioritize, and complete tasks    

You’ll be great for this role if:
•    Experience with Medicare STAR Metrics and HEDIS Measures
•    Experience in clinical/medical settings
•    Experience using EHR systems
•    General understanding of medical conditions and specialties

Environmental Job Requirements and Working Conditions:    

  • Our organization follows a hybrid work structure where the expectation is to work both in office and at home on a weekly basis, after a 90 day in-office training period. The office is located at 568 W. Garvey Ave., Monterey Park, CA 91754.  
  • The total compensation target pay range for this role is: $18.00 - $21.50 per hour. The salary range represents our national target range for this role.
