Job Title: Transfer Coordinator
Department: Health Services - ICM

About the Role:
The Transfer Coordinator is expected to perform a variety of duties, working closely with the Astrana Health On-Call Hospitalist in order to provide the highest quality service to our members. On Call Coordinator will provide and coordinate information with outside providers, i.e Emergency Rooms, PCP’s and offices, skilled nursing facilities, urgent care centers, outpatient surgical centers, home health, hospice, ambulance, etc.      
   
What You'll Do:      
  • Comply with CM policies and procedures.
  • Will coordinate with hospitalists to transfer of any ASTH IPA that manages dual/global risk member(s) in outside hospital emergency department back to in-network facilities.
  • Responsible in answering all inbound calls during after office hours from after hour on-call hotline numbers during after business hours Monday – Friday after by 5 pm & after including weekend and holiday as routinely assigned schedule by supervisor or manager
  •  Responsible for logging all types of calls in detailed in NMM daily on call log and submitting this log on a daily basis the next business day
  • Facilitate the communication between assigned ASTH IPA Hospitalist with the outside hospital ER MDs, urgent care, SNFs, Specialists and PCPs via telephone conference or using a secured messaging system to all parties
  • Coordinate with the all parties (MDs, hospital bed control, transportation & nurses) for a safe transfer of the member to the assigned in-network facility
  • Responsible in notifying the bed control such as AHMC Central Command Bed Control & others, via telephone call or through a secure messaging system for possible transfer once accepted by ASTH IPA Hospitalist
  • Responsible in arranging and coordinating transportations i.e. non-emergency ambulance when transferring members from outside hospital facilities, emergency department and others. Each hospital ER department follows different protocols when calling ambulance transport. Their protocol has to be followed
  • Must monitor closely that transfer is completed, and member had arrived in the new acute settings
  • Must notify the accepting ASTH IPA On-Call Hospitalist of ETA as well as room/bed assignment that member had arrived and assigned to room and bed number
  • Must involve on call/back up Case Manager for a complex coordination of care
  • Responsible for endorsing all pending transfers or issues to the incoming on call transfer coordinator
  • Must request copy of ED/Triage report via secured fax line or other IPA secured fax line number, if a member is not stable for transfer as confirmed by ASTH IPA On-Call Hospitalist
  •  Submit log and monthly bill for approval and payment
  • Maintains strictest confidentiality at all times
Qualifications:     
  • High School Graduate or equivalent
  • Knowledge of medical terminology, RVS, CPT, HPCS, ICD-9 codes
  • Proficient with Microsoft applications’ and EZCAP
  • Good organizational skills
  • Good verbal and written communication skills
  • Must have the ability to multitask and problem solve in a fast-paced work environment

You're great for this role if:   

  • Has 2 years of experience in Hospital ER Unit Secretary, a health care provider coordinator i.e., home health agency, a managed care environment to include but not limited to an IPA or MSO preferred

Environmental Job Requirements and Working Conditions:     
  • Our organization follows a hybrid work structure where the expectation is to work both in office and at home on a weekly basis. The office is located at 1600 Corporate Center Dr., Monterey Park, CA 91754.    
  • The total compensation target pay range for this role is: $18.00 - $25.00 per hour. The salary range represents our national target range for this role.
