Title: Hospice Certified Home Health Aid (CHHA) - Hybrid
Department: Quality – Hospice
About The Role:
We are seeking a compassionate CHHA to provide personal care services to the terminally ill hospice patient population, provide compassion and understanding to the family in times of bereavement, and work as a solid member of the Hospice team in providing end of life care according to guidelines and care plans.
What You’ll Do:
- Assist hospice patients in completely ADLs under the direction and ongoing supervision of the registered nurse.
- Complete accurate charting regarding changes in patient condition and keep overall accurate record of patient visits.
- Provide emotional support and compassionate care to the patient and family as a unit.
- Report any changes in patient condition or family situation by calling Hospice office and notifying the overseeing RN.
- Practice standard precautions according to OSHA regulations and policy.
- Adhere to all Hospice department policies and procedural guidelines.
- Maintain current CHHA certification as required by the state of CA.
Qualifications:
- CHHA in CA
- Ability to work independently and think critically.
- Acceptance of and adaptability to different social, racial, and religious ideologies.
- Valid CA Driver’s license and reliable transportation.

Environmental Job Requirements and Working Conditions:
- Our organization follows a hybrid work structure where the expectation is to work both in the office, in the field, and at home on a weekly basis. The office is at 1668 S. Garfield Avenue, Alhambra, CA.
- This is a full-time position, M-F 9A – 5 P.
- The target compensation for this position is $22 - $26 per hour.
