About the Role:     
We are currently seeking a highly motivated Pharmacy Technician. This role will report to an ACM Supervisor and enable us to continue to scale in the healthcare industry.  
  
What You'll Do:   
  • Responsible for member/provider outreach and notification (telephonic and/or written) utilizing department templates.
  • Contact members after hospital discharge to perform and complete medication reconciliation.
  • Establish rapport with member to obtain medication history or determine if member or family member can provide medication history.
  • Identifies medication names, common dosages, dosage forms, and frequency of administration for all medications.
  • Educates member/caregiver on medication safety and instructions.
  • Assist members in understanding medication instructions and usage by answering questions and addressing concerns.
  • Navigate electronic system and enter new medication history or if available update existing medication profile.
  • Assist with data entry, including entering member information and prescription details into the system.
  • Handle phone calls from members and healthcare providers.
  • Calls outpatient pharmacies, primary care office, or other healthcare facilities to confirm medication prescriptions or investigate discrepancies when necessary.
  • Verifies accuracy of medication history with member confirmation.
  • Consistently documents activities and interventions provided to the member according to the training.
  • Attends/participates in interdisciplinary team meetings.
  • Provides enrollee with support telephonically.
  • Be available to participate in on-call schedule.
  • Attends/participates in staff meetings.
  • Participates in all health plan audit and preparation as applicable.
  • Participates in all health plan seminars and training as applicable.
  • Develop a working relation with internal departments to maintain the flow of needed information.
  • Submit reports of productivity as applicable.
  • Adhere to all legal and regulatory requirements for pharmacy operations.
  • Ensure compliance with HIPAA and other privacy regulations.
  • Maintains the member’s right to privacy and protects Astrana Health operations by keeping information confidential.
   
Qualifications:   
  • High school diploma or equivalent
  • Certification as a Pharmacy Technician (CPhT)
  • Previous experience in a pharmacy setting.
  • Strong attention to detail and accuracy.
  • Excellent communication and interpersonal skills, with the ability to work with patients from diverse backgrounds.
  • Ability to keep a high level of confidence and discretion when dealing with sensitive matters relating to providers, members, business plans, strategies and other sensitive information is required.
  • Proficient with Microsoft Office (Word, Excel, Power Point)  
   
You're great for this role if:   
  • Experience with Medication Reconciliation Post-Discharge (MRP) HEDIS measure, including familiarity with the documentation and process involved in ensuring medication accuracy post-hospitalization.
  • Experience with post-discharge outreach to ensure patients understand their medication regimen and follow-up care.
  • Previous experience in managed care environment to include but not limited to health plan, IPA or MSO.

Environmental Job Requirements and Working Conditions:  
  • Our organization follows a hybrid work structure where the expectation is to work both in office and at home on a weekly basis. The office is located at 1600 Corporate Center Drive, Monterey Park, CA 91754 
  • The total compensation target pay range for this role is between $18.00 - $20.00 per hour. The salary range represents our national target range for this role.
