  1. Essential Duties and Responsibilities (Other duties may be assigned)
  • Analyzes and organizes home health operations for region of responsibility.
  • Participates in the development of identifying future opportunities for home care specialization
  • Monitors reports, metrics and progress towards goals established for the region and / or individual locations
  • Serves as a resource to guide and direct regional service delivery.
  • Represents the organization as a liaison with physicians and local community businesses.
  • Communicates as needed with the Director of Nursing concerning new developments and/or problems.
  • Responsible for review of Clinical Case Managers performance to include:
    • starts of care
    • timely re-certifications and authorizations
    • adequate and appropriate supervision of clients
    • effective billing controls
    • care delivery and
    • documentation requirements
    • Personnel annual performance review and competency completion
  • Assesses and assures Quality Improvement initiatives and any corrections and or Performance Improvement cycles are followed through
  • Participates in the interviewing, hiring, and training of location leadership.
  • Serves as an expert clinical resource to guide high quality training and competency assessments of care delivery staff.
  • Stay current on regulatory rules for services delivered to guide compliant performance.
  • Communicates and holds accountable local leadership for corporate projects or initiatives.
