Summary of Position: Assists Nurse Practitioners, Physicians Assistants, and/or Nurses, by performing administrative and clinical duties under the direction of a Registered Nurse. Duties include but are not limited to: receiving incoming calls and making outbound calls, developing positive relationships with nurses and families, working closely with clinical staff to provide the best care for our clients and maintaining a positive attitude. Administrative duties may include outreach to families, patient data collection and analysis, scheduling appointments,  coordinating calls between agency clinical staff and physician's offices, and follow up calls to clients and physicians. Clinical duties may include, recording patient medical history and current medical concerns.

Qualifications/Requirements: 
  • Bilingual: Fluent in English and Spanish  
  • Minimum Education: High School Diploma or equivalent 
  • Minimum Experience: 2 years minimum 
  • Minimum Certifications and/or Licenses: CPR and first aid certification required; certification as a medical assistant (CMA) preferred 
  • Minimum Skills: Experience with EMR, MS Word, Excel, MS outlook

Physical Requirements: 
  • The Triage Team Care Coordinator will be assigned a workstation, computer terminal, and telephonic equipment in a regular office environment. OR provided with a computer terminal and telephonic equipment for home office, work from home. You must have good manual and finger dexterity, excellent verbal and written communication, hearing and visual acuity and color distinction. The nurse must sit in front of a computer screen for prolonged periods of time.

Pay Range: $18 - $20 per hour
