We are seeking a Part-Time Licensed Clinical Social Worker (LCSW-R)

This position would be best suited for a person who appreciates cordial, professional employee relationships and close continuity of care with our families and children.  The ideal candidate will be willing to work with clinical and operational leaders to build the program to support additional offices in the future.

The Social Worker will work approximately sixteen to twenty (16-20) hours per week. For this position, availability and flexibility are very important because your schedule may vary from week to week depending on the needs of our patients and families.

Your projected weekly schedule is as follows:

Monday: 2:30pm – 6:30pm

Tuesday: 2:30pm – 6:30pm

Alternating Friday: 2:30pm – 6:30pm

Alternating Saturday: 8:00am – 12:00pm

Preferred Qualifications for Ideal Candidates:

  • Master’s Degree in Social Work
  • NYS Licensed Clinical Social Worker (LCSW-R)
  • Minimum of two (2) years’ experience providing direct services to children with mental illness, developmental disabilities, or care coordination in another capacity
  • Ability to communicate and interact effectively with patients and families of diverse cultural backgrounds
  • Experience and/or training preferred in commonly accepted treatment modalities.
  • Familiar with the concept of social determinants of health and ACEs (Adverse Childhood Experiences)
  • Bi-Lingual in Spanish, a plus
  • Must be able to travel to multiple physician practices and Home Office as required

Position Responsibilities:

  • Provides Clinical Behavioral Health Services to pediatric patient population while optimizing patient’s access to healthcare
  • Provides psychotherapy and care coordination services to patients with behavioral health problems which are treated in the medical home. Experience with evidence-based treatments is preferred, as is experience with parenting work and treatment of behavioral health conditions commonly seen in general pediatric practices
  • Will conduct diagnostic/intake evaluations of pediatric patients and provide individual treatment planning with patients and families
  • Coordinates referral services for patients as needed, including those who require a higher level of care. Inform patients, providers and other stakeholders of available services and the advantages provided by their use
  • Functions effectively as a part of a multi-disciplinary team in order to provide optimal care to patients and their families
  • Serves as a point-of-contact to patients, family members and caregivers. Encourages use of program resources to maximize wellness and independence
  • Performs administrative assessment of patient’s eligibility for social services and care management programs
  • Participates in community outreach and enrollment activities
  • Participates in developing relationships with referral sources to facilitate coordinated care.
  • Provides sensitive, culturally diverse assistance/guidance to patients, family members and caregivers
  • Assists patients in scheduling appointments and transportation for consultations, diagnostic testing, program meetings, etc.
  • Collaborates with clinical and non-clinical team members to maintain proper workflows, procedures, reporting and documentation
  • Maintains integrity of related records in accordance with system policies and procedures and State and Federal regulations
  • Utilizes Electronic Medical Records and interviews patients, family members and caregivers to compile and document relevant information (demographic, economic, social, institutional, etc.)
  • Performs related duties, as required

The Location: This position is based out of our Peds First Pediatrics Division located in Medford, NY.

Peds First Pediatrics has been in practice for 10 years serving the Medford community and surrounding areas. All medical services are performed under the supervision of Board Certified Pediatricians who are committed to working as a team, along with their staff, to ensure the best care if offered to all families.
