Community Habilitation Team Leader (On-Site, Middletown, NY)

Location: Middletown, NY
Salary: $25.00 per hour
Hours: Monday-Friday, On Site, 8:30AM-5:00PM (40hrs)

Job Description:
Primary duties include:
- Responsible for supervising and training staff
- Attend regular Life Plan meetings, review, and create Staff Habilitation Plans
- Track referrals and complete intakes for new cases
- Assist in monitoring quality of services for program participants
- Collaborate with teams to ensure appropriate budget management for people living independently
- Develop internal and external collaborative relationships with colleagues
- Assist in tracking and implementing appropriate strategies related to department unit utilizations
- Write formal staff evaluations, provide feedback to staff, and offer mentoring opportunities to allow staff to grow and achieve agency outcomes
Qualifications:
- 5-7 years related experience with people with intellectual and developmental disabilities
- 3 years of related supervisory experience
- Experience with creating and implementing new methods
- Candidate will be organized and precise and have exceptional time management skills
- Candidate must be able to lead and work effectively in a team environment and independently when necessary.
- Bi-lingual preferred not required
Education:
- Bachelor’s degree in human services field preferred
