Outreach Certified Peer Specialist – ACT Team (Westchester County, NY)

Join our recovery-focused ACT Team and make a difference!

We’re seeking an Outreach Certified Peer Specialist to support individuals living with chronic mental illness throughout Westchester County. This role uses lived experience to inspire hope, teach coping skills, and connect people to community resources that promote wellness and independence.

Key Responsibilities

  • Provide outreach, support, and counseling in the community and at client homes (up to 80% field-based).
  • Share personal recovery experience to educate and mentor clients.
  • Teach wellness and symptom management skills.
  • Advocate for recovery-focused services and help clients access benefits and supports.
  • Document progress and collaborate with the multidisciplinary ACT Team.
  • Responsible for on-call shifts monitoring and responding to after-hours calls to the program emergency phone.  Shifts are for one week at a time on a rotating basis

Qualifications

  • NYS Certified or Provisional Peer Specialist required.
  • Lived experience with mental health recovery required.
  • Strong communication, organization, and teamwork skills.
  • Electronic health record experience preferred.
  • Valid NYS Driver’s License required.
  • Bachelor’s Degree preferred; High School Diploma or equivalent required.
